Dose flags and allergy checks

Summary

Select Check references in the composer. The allergy check compares the drug you typed against this patient's own recorded allergy list. The dose references read age-banded caps, weight bands and no-guidance floors, and report how old the recorded weight is. Both flag and show what they read. Neither blocks, nothing is corrected for you, and an override is a normal outcome.

Before anything else

Nothing you draft or sign in Gale reaches a pharmacy today: Gale is not connected to the pharmacy network yet. A script you draft and sign exists only in this chart. Record it in Gale so the chart and the note are right, then send it by the route your practice already uses.

What prescribing does not do yet is the full list.

The allergy check reads this chart

It compares the drug you typed against this patient's own recorded allergy list. That is Gale's own data, so this check needs no licence from anyone and runs today. Three ways, strongest first, at most one flag per recorded row:

  • the recorded substance is the drug you typed;
  • the recorded substance appears as a named component of the string you typed. That is a claim about the text, not a formulation lookup, and the flag says so in those words;
  • a documented class relation, and only where that relation carries a source. Every row names its source.

The allergy list has been on the chart all along. Until this shipped, nothing read it.

The dose references

Check references reads a vendored table and reports what it found beside the dose you entered:

  • age-banded maximum daily caps, which are not monotonic — an older band can allow less than a younger one, so read the band rather than assuming the direction;
  • weight-based ceilings, for a short list that deliberately excludes stimulants;
  • weight-band selection, where a label's guidance differs above and below a threshold;
  • no-guidance floors, where the label says nothing at all for a patient that small;
  • how old the recorded weight is, in days. A fact, not a verdict;
  • growth velocity between two dated weights. No percentile is computed, because that needs a reference table Gale does not hold.

Your dose is echoed back unchanged. Nothing is substituted and nothing is rounded.

Why nothing blocks

No path in either check blocks, and there is no setting that makes one.

Pediatric dose alerts run heavily false-positive in the published literature, and an alert that blocks is an alert people learn to click through — the habit is what kills the one alert that mattered. A hard block would also be Gale deciding, and Gale is software: a licensed prescriber prescribes, under their own licence.

So an override is a first-class outcome, not a failure. Record your rationale when you override one. The trail then shows a clinician deciding, which is the whole reason it exists.

What is not checked

Drug-to-drug interaction is not checked. It comes back as not checked, naming the licensed source Gale does not hold. A clean screen is not a clear interaction check.

A hand-built interaction table is the one thing worth refusing to ship here: a wrong interaction verdict is worse than none, because it is the one that gets believed. What prescribing does not do yet lists the rest.

Common questions

Neither check blocks, by design. They flag and show what they read; the decision stays with you.

No. It is an expected outcome and it is recorded as one. Record your rationale with it, which is what makes the trail readable later.

No. Drug-to-drug interaction comes back as not checked and names the licensed source Gale does not hold. Allergy checking is a different check and it is real.

Yes. It reports the weight's age in days as a fact. It does not decide what that means for the dose.

It means nothing matched what is recorded on this chart. That is a statement about the record, not about the patient.
