Questions That Reveal a Clinic's Lab Quality

A clinic's marketing shows you the waiting room and the success headline, not the lab bench where the actual work happens. A short list of specific questions — about accreditation, thaw-survival rates, how outcomes are counted, and which extras are sold — separates a lab that runs on evidence from one that runs on packages. Here is what to ask, and why each answer matters.

Why the lab, not the brochure, decides the outcome

In vitro fertilization is named for the laboratory, because that is where the work happens: fertilization, embryo culture, freezing, and thawing all take place at the bench, out of the patient's sight. The physician manages stimulation and performs the retrieval and transfer, but embryologists handle the cells during the days that matter most. Two clinics with equally warm doctors can run very different labs.

That is why the embryology lab deserves its own set of questions, separate from how much you like the physician. Understanding why the ivf lab matters most reframes the comparison: a pleasant consultation cannot tell you whether embryos survive the freezer or grow well in culture. The lab is the part of a cycle you cannot see on a tour, so you have to ask about it directly.

Is the lab accredited, and who directs it?

Ask whether the laboratory is accredited, by whom, and who serves as its director. United States reproductive labs are commonly described as operating under federal CLIA lab standards, and many also carry voluntary CAP reproductive lab accreditation through the College of American Pathologists, which adds its own inspection requirements. A clear answer names the accrediting bodies and an experienced director.

Lab accreditation is a floor, not a ceiling — it confirms basic competence rather than excellence. Still, a clinic that cannot readily say who inspects its lab or who runs it has told you something. These are fair questions to raise at a first visit, alongside your other first-consult questions.

How does the lab report its own outcomes?

Ask how the clinic reports its results and against what benchmark. Since 1992, federal law has required fertility clinics to report their ART success rates, and the CDC publishes them 1. Ask how often the lab transfers a single embryo and how it decides. Multiple gestation — twins or more — is the principal avoidable risk of fertility treatment, carrying higher rates of preterm birth, low birth weight, preeclampsia, and gestational diabetes, and it is reduced by transferring fewer embryos 3. A lab confident in its culture and freezing can lean on elective single-embryo transfer and still succeed.

A clinic that routinely transfers two embryos may be buying a higher headline pregnancy rate at the cost of twin pregnancies. Which add-ons do you offer, and what is the evidence?

Ask which laboratory add-ons the clinic recommends and what evidence supports each. The UK fertility regulator rates common add-ons — assisted hatching, endometrial scratch, time-lapse imaging, and others — on a five-tier color-coded scale, and most sit at no good evidence of improving the chance of a live birth, with some flagged as potentially harmful 4. None of these has a single right answer, but the willingness to answer plainly is itself the signal.

Common questions

Many labs limit access because the culture environment is tightly controlled — temperature, air quality, and sterility all matter for embryos. A clinic may decline an in-person tour for that reason, which is not itself a red flag. What matters more is whether staff will describe the lab's accreditation, freezing methods, and outcomes clearly when you ask.

Not reliably. Newer equipment and marketed add-ons do not guarantee better live-birth rates, and many extras lack good supporting evidence. A lab's real quality shows in its outcomes reported by age band, its thaw-survival rate, and its willingness to favor single-embryo transfer — not in the length of its technology list.

It is worth asking why. A well-run lab typically tracks how many embryos survive thawing and can give at least a general figure. An inability or unwillingness to describe that number does not prove poor quality, but it removes a piece of information you would otherwise use to compare clinics. Note it and weigh it with everything else.

The physician oversees medications, performs the egg retrieval, and does the embryo transfer. The embryologists work between those steps — fertilizing eggs, growing embryos over several days, grading them, and freezing and thawing them. You will rarely meet the lab team, which is exactly why their track record has to be asked about rather than observed.

Sometimes, for specific situations rather than as routine upgrades. Some techniques have a clear role for particular diagnoses, while many are marketed broadly despite thin evidence. The useful question is not whether an add-on exists but whether there is good evidence it would help someone in your specific circumstances — and whether the clinic can point to that evidence.

When to seek help during an IVF cycle

  • Severe abdominal pain and bloating, rapid weight gain, persistent nausea, or shortness of breath in the days after an egg retrieval — signs of ovarian hyperstimulation syndrome that need same-day medical attention.
  • A clinic that quotes a single success percentage and will not show its age-specific or per-transfer numbers, or its own results next to national data.
  • Pressure to buy a bundle of lab add-ons before your own diagnosis and prognosis have been clearly explained.
