Eight regions cover Massachusetts, and three belong to other states too

The survey slices Massachusetts into eight regions: Amherst Town, Northampton; Barnstable Town; Boston, Cambridge, Newton; MA Rest of State; Pittsfield; Providence, Warwick; Springfield; and Worcester 1. That is unusually fine resolution for a small state, and it is the most useful thing this data offers a Massachusetts family — provided you read your own region rather than the state roll-up sitting on top of them.

Three of those eight also appear under other states. Boston, Cambridge, Newton sits in New Hampshire's list. Providence, Warwick sits in Rhode Island's — where it is that state's only region. Worcester appears in Connecticut's 1. Massachusetts does not have eight private regions. It has eight regions, three of which it shares.

The reason is structural rather than careless. Regions follow metropolitan statistical areas set by the federal Office of Management and Budget 1, and southern New England's metros ignore state lines entirely. The survey is explicit that its 2024 boundaries reflect a July 2023 federal refinement and can often include counties from other nearby states 1.

A shared region means a shared number. When two states' pages quote different figures for what is physically the same market, at least one of them is describing something other than the place it names.

Nationally the survey publishes across 431 regions built on 383 metropolitan statistical areas 1. Massachusetts draws eight of those. Maine — a state with far more land and fewer people — draws four 1, which is why a Massachusetts figure can usually be pinned to a real market and a Maine figure often cannot.

Boston's region does not stop at the state line

It reaches into New Hampshire, and that has consequences in both directions. Because Boston, Cambridge, Newton appears in New Hampshire's region list as well as Massachusetts's 1, a family in southern New Hampshire and a family in Newton may be reading a figure drawn from the same pool of providers. The border is a tax boundary, not a care market.

What sets the number is wages, and wages are metropolitan. The survey attributes the 2024 increases to inflation and labour costs in roughly equal measure, and names labour as the leading driver for home care 1. Communities inside the Boston orbit compete for aides against every other employer in that orbit. A community twenty minutes over a state line competes in the same contest.

Why this matters when you are comparing options. Families near a border often assume crossing it will find a cheaper market. Sometimes it does. But if both addresses fall inside the same survey region, the published figures will not show a difference, because there is no difference to show — it is one region reported twice.

If a nearby state's page quotes a number close to your own, that is not a coincidence to puzzle over. You may simply be looking at the same region wearing a different label.

The corollary is that a genuine price difference across a border shows up only when the two addresses sit in different regions. That is checkable in a minute, and it is worth checking before anyone drives anywhere.

Barnstable and Pittsfield are surveyed as their own worlds

Two of the eight regions exist for places that most states would have folded into a residual. Barnstable Town — the Cape — has its own region. So does Pittsfield, in the Berkshires 1. Neither is averaged into Boston, and that single decision makes the published figures usable for two of the hardest markets in the state to place someone in.

This is quietly important. The Cape and the Berkshires are not suburbs and do not behave like them. They have their own labour markets, their own seasonal economies, and their own scarcity of dementia beds. A survey that folded them into a Boston average would produce a figure that misled the families who most need a reliable one, because they are the families with the fewest options within driving distance.

Springfield and Amherst-Northampton are also split apart 1, which is finer resolution than the Pioneer Valley usually receives. Between them, these four regions mean western and coastal Massachusetts are described on their own terms rather than through Boston's.

MA Rest of State still exists, and it catches whatever the seven named regions do not reach 1. It is a smaller residual than most states carry, but the same caution applies: if your address lands there, the figure you are reading is a blend rather than a market.

The cost the map never shows. A Cape family choosing a community off-Cape for a lower rate is buying a bridge crossing every visit. A Berkshires family looking east is buying an hour each way. That trade is invisible in a median and decisive in practice — the visits are the entire point of the placement, and they are the first thing that quietly stops happening.

Memory care is not a category the survey prices

This is the sentence most Massachusetts cost pages will not tell you. The Cost of Care Survey collects rates in four categories: home care, adult day health care, assisted living communities, and nursing homes 1. A dementia unit is not one of them. There is no memory care median for Massachusetts. There is no memory care median for anywhere.

So when a page quotes a precise Massachusetts memory care figure, it is reporting someone's arithmetic, not a measurement. The usual method is to take the assisted living number and apply a markup chosen by whoever is writing. No survey stands behind that markup.

The two categories that do exist give you a bracket to think inside. The 2024 national assisted living median was $5,900 a month, up 10 percent from $5,350 the previous year 1, which annualises to about $70,800 2. Nursing homes — described by the survey as providing a higher level of supervision and care than assisted living — ran $305 a day semi-private and $350 a day private 1, roughly $111,325 and $127,750 a year 2. Providers reported all of it between July and December 2024 1.

A dementia unit prices somewhere between those two rungs. That is the honest resolution available. Anything more precise, from any source, is invention.

The median is a midpoint, not an entry rate

One line of methodology reframes the whole assisted living figure. Surveyors gathered monthly private-pay rates as they ranged from basic care to more substantial care for a one-bedroom unit, and when a community gave a range, the survey used the average of the high and the low 1.

The median is therefore the middle of a span, not the bottom of one.

Basic care to substantial care is the span a single community charges across, depending on how much help a resident needs. The published median collapses that span to its midpoint. Dementia-level needs sit toward the substantial end — above the midpoint, by a margin the survey does not quantify 1.

Care need is what the money buys, and the federal data supports that reading. Residential care community residents' characteristics — including whether they have a diagnosed dementia and how much help they need with daily activities such as bathing, dressing and transferring — vary systematically with the size and type of community 3. The premium is staffing, not a locked door.

Which is also why the same resident's price climbs without moving rooms. Dementia progresses 4. A care level assessed at move-in describes a person who will not exist in a year, and the rate follows the person rather than the apartment. Families frequently read this as a bait-and-switch. It usually is not; it is the assessment doing exactly what it was built to do, on a schedule nobody discussed at the tour.

Where home care stops being the cheaper option

Many families try to hold the line at home, and for a stretch that is both cheaper and better. The survey's own conventions show where the arithmetic turns, and the turn arrives earlier than most people expect. It turns at the point where the hours needed stop being a few and start being most of the day.

In 2024 the national median for a home health aide was $34 an hour, up 3 percent from $33; homemaker services ran $33 an hour, up 10 percent from $30 1. The survey annualises home care on a convention of 44 hours a week across 52 weeks 1.

Run that convention out. Forty-four hours a week of aide time at the national median is roughly $77,792 a year — more than the $70,800 median for a full year in an assisted living community 2.

On the survey's own annualisation, 44 hours a week of home health aide time already costs more per year than the median assisted living community 12. And 44 hours a week is about six hours a day — nowhere near what advancing dementia needs.

That is the crossover, and it is why home care so often stops being the economical choice precisely when it stops being sufficient. The hours required rise as supervision needs rise, and the cost rises linearly with them. A community's rate does not.

Adult day health care is the rung people forget. The 2024 national median was $100 a day, up 5 percent from $95, annualised by the survey at five days a week over 52 weeks 1 — about $26,000 a year. For a family still managing at home with a caregiver who needs to keep working or simply needs to sleep, it is the cheapest structured option on the board and it is frequently overlooked until it is too late to help.

The one-time fee, and the naming problem

Before any monthly rate begins, most communities want a one-time payment. The survey found approximately 58 percent of assisted living communities charge a one-time, non-refundable community or entrance fee 1 — near enough three in five. It is due up front, it rarely appears in the brochure's headline, and non-refundable means what it says.

That wording carries real risk in a dementia placement. Some placements fail in the first weeks: the person cannot settle, or the assessment understated them and the community decides it cannot meet the need. A non-refundable fee paid into that outcome is simply spent.

Shopping is harder than it should be because the vocabulary is not standardised. The survey counts more than 70 different names or designations for facilities licensed as some form of assisted care community, and reports that fewer than 40 percent use "assisted living" in their formal name or licensure — some are designated residential care instead 1. Two Massachusetts buildings offering comparable dementia care may not surface under the same search, and their fee sheets may not line up line for line.

A community that supplies all six is behaving normally. One that cannot has answered a different question.

When the money runs out

This deserves attention long before it is close, because the sums are large and the paperwork is slow. Against a national assisted living median of about $70,800 a year 2 — with a dementia unit above it — a sum that felt like security turns into a countdown. There is no end date to plan toward: an estimated 6.9 million Americans aged 65 and older were living with Alzheimer's dementia in 2024, and the disease progresses 4.

Medicaid is the long-term care programme; Medicare is not. Eligibility is means-tested, and the rules on assets, transfers and look-back periods are hostile to improvisation and to late discovery. Specialist advice, obtained early, generally pays for itself several times over.

Estate recovery is the federal requirement that states recover certain Medicaid long-term care costs from the estates of the people who received that care, after their death 5. It is why the family home so reliably becomes the centre of the discussion, and why the discussion goes better years ahead of the crisis than inside it.

PACE is worth a phone call. The Program of All-Inclusive Care for the Elderly provides all Medicare- and Medicaid-covered services plus anything the interdisciplinary care team decides a participant needs, and those enrolled with Medicaid generally pay no monthly premium and no cost-sharing for PACE-approved care 6. Its purpose is keeping someone in the community rather than in a facility, so it does not fit every case, and it runs only where a programme exists. For a family on the Cape or in the Berkshires, whether a programme reaches your address is a short question with an occasionally decisive answer.

Running out is not evidence that anyone planned badly. It is the ordinary arithmetic of a long dementia meeting an ordinary set of savings.

No. Memory care is not among the survey's four categories — home care, adult day health care, assisted living and nursing homes. No memory care median exists for any state. Specific Massachusetts figures you find are markups someone applied to the assisted living number, with no survey standing behind the markup.

Not for the part that dominates the bill. Medicare does not cover long-term custodial care — the room, board and daily supervision a memory care community exists to provide. It covers medical care, and a limited skilled nursing benefit after a qualifying hospital stay. Medicaid is the long-term care programme, and it is means-tested.

The care assessment probably changed. Dementia is progressive, so needs measured at move-in will not describe the same person a year on, and communities reassess and reprice accordingly. Annual increases apply on top of that. Asking for a building's three-year increase history surfaces both mechanisms before you sign anything.

When cost is no longer the pressing question

  • —A person with dementia has left alone and cannot be located, or has been found near water, traffic or a rail line — much of coastal and metro Massachusetts puts all three within a short walk

If someone cannot be woken, is struggling to breathe, or has fallen and hit their head, call 911 or go to an emergency department.
