Why Maine has no published memory care rate

Because memory care is not a category anyone surveys. The Cost of Care Survey — the source nearly every Maine cost page quietly reprints — collects rates in four categories: home care, adult day health care, assisted living communities, and nursing homes 1. A dementia unit is not among them. Every "average memory care cost in Maine" figure circulating online is therefore an extrapolation someone built on top of the assisted living number, not a surveyed price.

That matters more than it sounds. An extrapolation carries no error bars and no method, and it cannot be checked. When a page tells you memory care in Maine runs a specific dollar amount, no one measured that. Someone took the assisted living figure and applied a markup they chose.

What the survey does measure gives you two real anchors. Nationally, the 2024 median for an assisted living community was $5,900 a month, up 10 percent from $5,350 the year before 1 — about $70,800 a year 2. A nursing home, which the survey describes as a higher level of supervision and care than assisted living, ran a median $305 a day for a semi-private room and $350 a day for a private one 1, or roughly $111,325 and $127,750 a year 2. Those rates were collected from providers between July and December 2024 1.

A dementia unit in Maine sits somewhere between those two rungs, and the survey does not tell you where. The rest of this page is about closing that gap with the community's own numbers rather than a stranger's arithmetic.

The four regions Maine is surveyed in

Maine is covered by exactly four survey regions: Bangor; Lewiston, Auburn; Portland, South Portland; and ME Rest of State 1. That is the entire granularity available for this state. Nationally the survey publishes costs across 431 regions built on 383 metropolitan statistical areas 1, so some states are sliced fine and some are barely sliced at all. Maine is in the second group.

This is worth sitting with before you trust any single Maine figure. Three of Maine's four regions are the southern and central population centers. The fourth is everything else — and in a state shaped like Maine, everything else is most of the map.

Maine's state capital does not have its own survey region. Augusta is not among the four, which places it in the ME Rest of State bucket 1.

Regions are drawn from metropolitan statistical area definitions set by the federal Office of Management and Budget, not by the survey itself 1. Augusta and Waterville are not an MSA, so they fall through to the residual. The same is true of Bar Harbor, Rockland, Presque Isle, Calais, Millinocket and every other Maine community outside the Portland, Lewiston-Auburn and Bangor orbits.

The practical consequence: when you read a Maine number, ask which of those four regions it came from. A Portland figure and a Rest of State figure are describing different markets that happen to share a state line. What the ME Rest of State bucket hides

A residual bucket averages a real place with an unlike one. ME Rest of State blends Aroostook County with the midcoast, Washington County with the Kennebec valley, and the western mountains with Down East. These are not one market. They differ in what staff can be paid, how far a relief shift has to drive, and whether there is a second community within an hour to compete on price.

For a family in Fort Kent or Machias, that has a specific consequence. The published Rest of State number is an average that may not describe any community you can actually reach. Rural Maine memory care is thin on the ground, and thinness does not show up in a median — it shows up as a waitlist, or as a drive.

What to do with that. Treat the Rest of State figure as a rough centre of gravity and nothing more. The number that governs your decision is the one a specific community writes down for a specific person.

The distance cost nobody prices. If the nearest dementia unit is in Bangor and you are in Presque Isle, the survey figure is missing the real cost of that placement: the fuel, the winter driving, the visits that do not happen because it is three hours each way. Families consistently underweight this and then discover it in February. It is not a line on any invoice, and it is the thing that most often gets a placement undone.

There is a further wrinkle in how these regions are drawn. The federal delineations were refined in July 2023, and the 2024 data reflects the new boundaries, which can include counties from other nearby states 1. Maine's own regions sit wholly inside Maine, but the New Hampshire border markets are a reminder that a metro region is not a state.

Maine's median is a midpoint, not a starting price

This is the single most misread thing about the assisted living figure, and it is the closest the published data comes to explaining the dementia premium. Surveyors collected monthly private-pay rates as they ranged from basic care to more substantial care for a one-bedroom unit — and where a community gave a range, the survey used the average of the high and the low 1.

Read that again, because it changes what the number means.

The published assisted living median is not what a low-needs resident pays. It is the middle of a span running from basic care up to substantial care. Somebody with dementia-level needs sits at the top of that span, not at its centre 1.

So the Maine assisted living median is not a memory care price and it is not a floor for a person who needs real help. It is the midpoint of a range that already contains people needing far less than your parent does. Anyone who takes that midpoint and calls it "the Maine assisted living cost," then adds a markup to reach "the Maine memory care cost," has stacked one guess on top of a number they misunderstood.

The honest version is duller and more useful. The published figure tells you what order of magnitude Maine's market runs at, and whether it is moving — a 10 percent national jump in a single year 1 is the kind of movement that makes a two-year-old quote worthless. It does not tell you what your family will pay. Only an assessment does, and that is the next thing to ask for.

Why a dementia unit costs more than a standard apartment

The premium is not a surcharge for a locked door. It is staffing, and staffing tracks how much help a person actually needs. Federal data on residential care communities shows that residents' characteristics — including whether they have a diagnosed dementia and how much help they need with daily activities like bathing, dressing and transferring — vary systematically with the size and type of the community 3. Care need is what the money buys.

Activities of daily living, usually shortened to ADLs, are the basic self-care tasks — bathing, dressing, toileting, transferring from a bed to a chair, walking, eating. How many of these a person needs help with, and how much help, is what drives a care level and therefore a price.

A person in the middle of dementia may need someone within sight for most of the day, prompting through a shower rather than assistance in one, and two staff for a transfer. That is a different labour cost from a resident who needs a pill reminder at eight. The dementia unit charges more because it staffs more.

This is also why the same person's price rises over time in the same building, without anyone moving. Dementia is progressive 4. A care level assessed at move-in describes a person who will not exist in eighteen months. Families are routinely blindsided by this and read it as a bait-and-switch. It usually is not. It is the assessment doing what it was always going to do.

The question that surfaces it. Ask what triggers a reassessment, how much notice you get before a new rate starts, and what the largest care-level increase in that building has been in the last year. A community that cannot answer the third question is telling you something.

The fee that arrives before the first month's rent

Before any monthly rate applies, most communities charge a one-time entrance fee, and it is large enough to change which places you can consider. The survey found that approximately 58 percent of assisted living communities charge a one-time, non-refundable community or entrance fee 1. It is not universal, it is not usually advertised in the brochure, and it is rarely refundable if the placement fails in the first month.

That last clause deserves weight. Dementia placements sometimes do fail early — the person cannot settle, or the assessment underestimated them and the community says it cannot meet the need. A non-refundable fee paid in that scenario is simply gone.

Maine's licensing vocabulary makes this harder to shop than it should be. The survey notes there are more than 70 different names or designations for facilities licensed as some form of assisted care community nationally, and that fewer than 40 percent use the term "assisted living" in their formal name or licensure — some are designated "residential care" instead 1. So two Maine buildings offering comparable dementia care may not be searchable under the same word, and their fee structures may not be comparable line for line.

A community that will put all five in writing is not doing you a favour. It is doing the ordinary thing, and it is a reasonable minimum to expect.

What happens when the money runs out

Most families paying privately for memory care will eventually face this, and the time to understand it is before it is close. At a national median assisted living rate of roughly $70,800 a year 2 — with a dementia unit priced above it — savings that felt substantial can be measured in months, not years. Dementia is a progressive condition 4, which means the spend has no natural end date you can plan around.

Medicaid is the backstop, and it is not the same door as Medicare. Medicaid long-term care eligibility is means-tested, and the rules on assets, transfers and look-back periods are unforgiving of improvisation. Advice here is worth paying for, early, from someone who does only this.

Estate recovery is the federal requirement that states recover certain Medicaid long-term care costs from the estates of people who received them after their death 5. It is why the family home so often becomes the centre of the conversation, and why it should be discussed years before it is urgent rather than in a crisis.

PACE is worth knowing about. The Program of All-Inclusive Care for the Elderly covers all Medicare- and Medicaid-covered services plus anything the interdisciplinary care team decides a participant needs; people enrolled with Medicaid generally pay no monthly premium and no cost-sharing for PACE-approved care 6. It is built around keeping someone in the community rather than in a facility, so it does not fit every situation, and it operates only where a programme exists. The all-in monthly cost for your specific person, in writing, from the specific community: base rate for the dementia unit, plus the care level their assessment produced, plus anything billed separately. That figure is knowable and comparable. A state median is neither, and it is not what anyone will invoice you.
