The nose sits in the T-zone and has a high density of sebaceous (oil) glands. More oil production means more opportunity for pores to become plugged. Non-comedogenic moisturizer and sunscreen. Keeping skin well-moisturized does not worsen blackheads — in fact, skipping moisturizer can stimulate more oil production. Look for products labeled 'non-comedogenic.'

Harsh physical scrubs abrade the skin surface and can cause micro-tears that worsen inflammation without clearing pores. They can also trigger post-inflammatory hyperpigmentation, particularly in medium-to-darker skin tones.
Squeezing or picking with fingers risks pushing debris deeper, causing inflammation, and introducing bacteria that can turn a comedone into an inflammatory pimple or cyst.

Are those dots actually blackheads — or sebaceous filaments?
Sebaceous filaments are normal structures within pores — gray or tan pin-sized dots arranged symmetrically across the nose that return very quickly after extraction. They are not blockages in the way blackheads are. True blackheads are darker, may be slightly raised, and do not return to the same spot immediately after removal.
A dermatologist can distinguish these under magnification. Treating sebaceous filaments aggressively as though they were blackheads can cause unnecessary irritation without improving the result.
When does a dermatologist add value?

How long does it take for salicylic acid or adapalene to clear blackheads?
Consistent use typically produces noticeable improvement in four to eight weeks. Maximum benefit from retinoids like adapalene generally takes three to six months. Stopping and restarting products resets that timeline.
Can diet cause blackheads?
The evidence linking diet directly to comedones is mixed and less robust than for inflammatory acne. High-glycemic foods and dairy have been studied most; if you notice a consistent pattern between certain foods and breakouts, it is worth discussing with a dermatologist.
Is it safe to use adapalene every day?
Adapalene is generally well-tolerated with daily use, though some people start with every-other-day application to minimize initial dryness or irritation. A sunscreen in the morning is important when using any retinoid, as they can increase sun sensitivity.
Do blackheads need to be extracted?
Not necessarily. Consistent use of retinoids and salicylic acid will loosen and clear most blackheads over time. Professional extractions by a trained esthetician or clinician are safer than home extraction if manual removal is desired.
Can blackheads cause permanent scarring?
