How Long Is a Sports Physical Valid?

Does 'valid for one year' always mean 'accepted for one year'?

The one-year guideline is practical rather than strictly evidence-based — it assumes a year is a reasonable interval for catching changes in cardiovascular health, growth, or injury history. Most state high school athletic associations require an annual PPE for school-based sports participation 1.

But validity and acceptance are not the same thing. A school year running September through June might require a physical completed no earlier than the previous spring. Some states count by calendar year; others by academic year. Some governing bodies layer additional requirements on top of the school's rules.

The only reliable answer: ask the athletic director or school nurse for the exact window before you book the appointment.

What do the national guidelines say?

The fifth edition of the Preparticipation Physical Evaluation (PPE5), developed jointly by the AAP, AAFP, American College of Sports Medicine, and other organizations, recommends a full evaluation every two to three years with annual updates as needed 2. The full evaluation includes a comprehensive cardiac and musculoskeletal screen; the annual update revisits concerning findings from prior years.

The PPE5 also recommends the exam take place in the athlete's primary care medical home — during an office visit, not in a group setting — and suggests scheduling at least six weeks before the first preseason practice to allow time for follow-up on any findings 2.

What can make a physical effectively out of date before the year is up?

Even within a valid period, certain events prompt re-evaluation 2:

  • A new significant injury or surgery
  • A new diagnosis (heart condition, seizure disorder, concussion)
  • A major change in medications
  • Symptoms during or after exercise — chest pain, syncope, palpitations — that were not present at the last exam

Coaches and athletic trainers are often required by their institutions to flag these events and refer the athlete back for clearance, regardless of whether the original physical is technically still current.

When is the best time to schedule?

Many families schedule in late summer just before the school year. A crowded August calendar means longer waits and less buffer if the clinician finds something that needs follow-up before clearance is granted.

The PPE5 recommends scheduling at least six weeks before the first preseason practice 2 — typically spring for fall sports, early summer for winter sports — so there is time to address any concerns.

Which form should you bring?

  • Chest pain, fainting, or near-fainting during exercise — these should be evaluated before the physical, not waited on
  • Palpitations (heart racing or skipping) that occur during activity
  • A family member who died suddenly before age 50, especially from a cardiac cause
  • A prior concussion with symptoms that have not fully resolved
