Getting the School Physical Form Filled Out: Who Does What and When
A school physical form is completed and signed by a licensed clinician — a physician, nurse practitioner, or physician assistant — during an in-person exam. Parents bring the school's specific form to the appointment, the clinician performs the exam and fills in the medical sections, and the parent signs the consent portion.

A school physical — also called a well-child visit or pre-participation exam — includes a review of medical history, measurements (height, weight, blood pressure), vision and hearing screening, and an assessment of the heart, lungs, abdomen, and musculoskeletal development. The clinician documents findings, checks the required boxes on your form, and signs it. If everything looks normal, the form is ready to submit the same day.
How do I get the correct form before the appointment?
Many schools require their own district-specific form, not a generic one from the internet. Contact your school's front office, nurse, or website to download the current form before the appointment. Bringing the wrong form — or arriving without any form — typically means the clinician cannot certify it for your school. If your state uses a standardized form, your state health department's website may have it. Bring two copies when possible.
When should I schedule — and why timing matters?
Late July and August are the busiest period for school physicals. If your regular provider is fully booked, many urgent care centers and retail health clinics offer walk-in school physicals.
Can the clinician sign the form without seeing my child in person?
No. A school physical form certifies findings from a hands-on exam — it cannot be completed by telehealth alone. Clinicians also cannot backdate a form or sign based on an exam from a prior year. If your child has recently seen a specialist, bringing those records to the appointment provides helpful context, but the clinician still performs their own assessment.
What if the clinician finds something?
Occasionally a clinician identifies something that needs follow-up before clearing a child for sports or activities — a heart murmur, elevated blood pressure, or an orthopedic concern. This does not always mean disqualification; it means the finding needs further evaluation. The clinician will explain next steps. Most children are cleared without any issues.

Common questions
Is a sports physical the same as a school enrollment physical?
Not always. A sports physical focuses on musculoskeletal and cardiovascular fitness for athletic activity. A school enrollment physical has broader developmental and vaccination components. Some states accept one in place of the other; many do not. Clarify with your school before booking.
My child had a physical six months ago. Can the clinician just sign the form?
Generally no — the form certifies findings from a current exam, not an older one. Some clinicians may review and update recent records if the visit was recent enough, but this depends on your school's requirements and the clinician's judgment.
What if we cannot afford a private clinic?
Local health departments, federally qualified health centers, and school-based health centers often offer low- or no-cost school physicals. Medicaid and CHIP cover well-child visits at no cost for eligible children.
