Yes, once the National Registry listing is in hand. Two exceptions sit in the same section and neither one helps a generalist. But the federal half is the easy half. FMCSA does not decide whether your state authorizes you to perform physical examinations. It defers to the state that licensed you, and then audits your listing against that state's answer. So the question resolves in two documents: the National Registry rule, and your own nurse practice act.
That split runs through all three form families on this page. The DOT card has a federal gate on top of the state one. Sports physicals and disability placards have no federal gate at all, which makes them purely a state question with a different answer in every state.
The NRCME sequence, in the order the rule requires
Register before you train. Doing those first two steps out of order is the expensive mistake: the training has to be the training the rule describes, and the number has to exist before it starts.
The regulation fixes no fee for either the training or the test, so the number to budget against comes from the training provider's own published price and the testing organization's. It also fixes no timeline beyond that three-year outer window, so how long the sequence takes in practice is a scheduling question the CFR does not answer.
What keeps the listing alive once you have it
Continuing state authorization, periodic training, and documents produced on short notice. The credential itself runs 10 years. The per-state clause is the one that catches a solo practice that grows. An NP who adds a second state license, or who examines drivers across a state line, is carrying an authorization question in each of those states, and the audit windows are short enough that the documentation has to already be filed rather than assembled on request.
The forms a DOT exam produces, and the file you keep
Two forms and a three-year file. The examination produces the Medical Examination Report, Form MCSA-5875, and the Medical Examiner's Certificate, Form MCSA-5876. Sports physicals: no registry, one state statute at a time
Sports physicals carry no federal counterpart to the National Registry, so the signer list comes entirely from state law and differs by state. Florida is one worked example. Illinois answers an adjacent but different question, and the difference matters if you delegate. Neither statute tells you anything about a third state.
The lookup is short. Find the state statute or administrative rule governing school athletics or school health examinations, then find the state high school athletic association's current bylaws and its preparticipation evaluation form, then read the signature block on the form itself. In practice the form is the most specific document in the chain, because it prints the credentials the school will accept and the statute usually does not.
Disability placard certificates, and what California's version shows
A nurse practitioner may sign the disability placard certificate in California, and the grant is worth reading in full. The same section attaches two duties that outlast the appointment. But California's answer is California's. Placard certificates are creatures of state vehicle codes, and a state that has not written a nurse practitioner into its own has not authorized one by implication. The lookup starts at the state motor vehicle agency: pull the current placard application, read its certifying-provider block, and open the code section the form cites.
How to check your own state before booking the first appointment
Three documents answer the question, in this order. Start with the form itself and its signature block, because the issuing agency prints the credentials it accepts. Then read the statute or rule the form cites. State law layers separate authorizations on top of a license, and a license alone settles fewer questions than it looks like it should. The in-office dispensing permit is one of those layers. The National Registry listing is another. Holding an unrestricted license without the layer the form requires does not get the form signed.
