AI Medical Scribe, Included — No Monthly Fee
Gale bundles an AI ambient scribe — the Jefferson scribe — into the practice OS at no monthly fee. Competing standalone tools charge $39–$119/month (Freed), ~$150/month (Heidi), or $369+ per provider per month (Nuance DAX). JAMA data from five academic health systems found AI scribes reduce documentation time by ~16 minutes per eight-hour shift. Audio is deleted after transcription; an on-device option keeps recordings off the cloud entirely.

What an AI Medical Scribe Does
An AI medical scribe listens to the provider-patient conversation — passively, in the background — and drafts a structured clinical note. The output is typically a SOAP note (subjective, objective, assessment, plan) or a visit summary keyed to the provider's preferred format. The provider reviews and signs; the AI handles the transcription and structuring work.
The category emerged as a direct response to EHR documentation burden. A 2025 study published in *JAMA Network Open* 1Ref 1Tai-Seale M, Olson CW, Li J, et al. (2025).Primary care visits run a half hour. Time on the EHR? 36 minutes.36.2 minutes EHR time per 30-minute primary care visit; 6.2 minutes pajama time per encounter found primary care physicians spend an average of 36.2 minutes on the EHR per 30-minute visit, including 6.2 minutes of after-hours "pajama time" per encounter. Across a week, documentation displaces direct patient care and is among the leading drivers of physician burnout.
What the Evidence Says on Time Savings
The most rigorous AI-scribe time-savings data to date comes from a 2026 JAMA study 2Ref 2Rotenstein L, Mishuris RG, et al. (Mass General Brigham / UCSF) (2026).JAMA Study: AI Scribes Deliver Modest EHR Time Savings Across 5 Major Health Systems.16 minutes/shift documentation reduction; 13.4 minutes/shift EHR time reduction; power users (~50%+ adoption rate) saw 27.3 minutes documentation reduction; no significant change in after-hours EHR time at average adoption led by researchers at UCSF and Mass General Brigham. The study tracked 8,581 ambulatory clinicians across five academic health systems (Mass General Brigham, Emory, UCSF, Yale New Haven Health, UC Davis).
Findings:
- Documentation time fell by an average of 16 minutes per 8-hour shift for AI-scribe adopters 2Ref 2Rotenstein L, Mishuris RG, et al. - Scribe adopters saw approximately one additional patient per two weeks 2Ref 2Rotenstein L, Mishuris RG, et al. (Mass General Brigham / UCSF) (2026).JAMA Study: AI Scribes Deliver Modest EHR Time Savings Across 5 Major Health Systems.16 minutes/shift documentation reduction; 13.4 minutes/shift EHR time reduction; power users (~50%+ adoption rate) saw 27.3 minutes documentation reduction; no significant change in after-hours EHR time at average adoption
- After-hours EHR time ("pajama time") did not show statistically significant change at average adoption levels 2Ref 2Rotenstein L, Mishuris RG, et al. A separate analysis of The Permanente Medical Group's deployment 3Ref 3American Medical Association (2025).AI scribes save 15,000 hours — and restore the human side of medicine.15,791 physician hours saved over one year at The Permanente Medical Group; 84% reported positive effects on patient communication; 82% reported improved work satisfaction found the system saved an estimated 15,791 total physician hours over one year — equivalent to 1,794 eight-hour workdays — across more than 2.5 million patient encounters. In that cohort, 84% of physicians reported positive effects on patient communication and 82% reported improved overall work satisfaction 3Ref 3American Medical Association (2025).AI scribes save 15,000 hours — and restore the human side of medicine.15,791 physician hours saved over one year at The Permanente Medical Group; 84% reported positive effects on patient communication; 82% reported improved work satisfaction.
The honest read: average time savings are meaningful but more modest than early marketing implied. Providers who use the scribe consistently across most visits realize substantially more benefit than occasional users.
The Cost Gap Between Standalone Scribes and a Bundled OS
Standalone AI scribes are priced as subscription software-as-a-service. As of June 2026, the major independent options are:
- Freed 4Ref 4Freed (getfreed.ai) (2026).Freed AI Medical Scribe — Pricing.Freed tiers as of June 2026: Starter $39/mo (40 notes/mo cap), Core $79/mo (unlimited), Premier $104/mo annual or $119/mo monthly; 7-day free trial: Starter $39/month (capped at 40 notes/month); Core $79/month (unlimited notes); Premier $104/month billed annually or $119/month billed monthly (adds EHR push and ICD-10 coding). A 7-day free trial is available on all paid tiers.
- Heidi Health 5Ref 5Heidi Health (2026).Heidi Health Pricing Guide 2026.Heidi Clinician plan $150/user/month billed annually as of 2026; free tier limited to 10 Pro Actions/month; Enterprise custom: A free tier exists with 10 "Pro Actions" per month. The Clinician paid plan is $150/user/month billed annually ($1,800/year) — up from its prior $90/month annual price — for unlimited Pro Actions, custom templates, and priority support. Enterprise (custom) adds EHR push and BAA coverage.
- Nuance DAX Copilot / Dragon Copilot 6Ref 6Multiple independent pricing analysts (2026).Nuance DAX Copilot Pricing 2026: Real Costs.DAX list pricing $369–$600/provider/month; setup fees $5,000–$50,000; 3-year contracts; 10+ provider minimums typical; rebranded Dragon Copilot March 2025: List pricing is approximately $369–$600 per provider per month. New deployments include one-time setup fees reported at $5,000–$50,000 depending on EHR environment and practice size. Most contracts require a 3-year term and a minimum of 10 providers, making DAX impractical for independent practices.
The Jefferson scribe, bundled in Gale, costs $0/month as a line item. The scribe is part of the practice OS — not an add-on. There is no subscription tier required to unlock it and no per-note fee.
Estimated annual scribe cost for a solo clinician:

- Jefferson scribe via Gale: $0/year in scribe fees [note below]
Gale is pre-commercial and in a synthetic-demonstration phase as of June 2026. No real patient money is currently processed. The above comparison reflects the model as designed — the software and scribe carry no subscription fee; Gale earns a percentage of billing collections only on claims that actually pay.
How the Jefferson Scribe Works
The Jefferson scribe records the provider-patient session and produces a structured SOAP note. Two paths are supported:
On-device (iOS, for providers using the Gale mobile app): Transcription runs on-device. The audio never leaves the phone. This is the highest-privacy path — there is no cloud transmission of the raw recording.
Web (browser-based, for the provider Grove surface): Audio is sent over an encrypted connection to the transcription service (Modal Jefferson-Whisper). The recording is deleted after the note is generated. The transcript is used to produce the note and is not retained for model training.
In both cases: the provider reviews and edits the generated note before it is signed and saved. The scribe produces a draft, not a final record. The provider's edit is the authoritative version — and in Gale's design, that edit becomes a training signal for improving note quality over time, with all personally identifiable information removed before any retention.
Jefferson notation preserves natural speech rhythm cues (pauses in conversation), which can surface clinically relevant patterns in provider-patient dialogue that flat transcription misses.
Privacy and HIPAA Considerations
Every AI scribe that processes patient audio is a business associate under HIPAA and requires a signed Business Associate Agreement (BAA). This is not optional. Tools that lack a BAA — including general-purpose AI assistants used off-label to process transcripts — are not HIPAA-compliant for identifiable patient data.
Key questions to ask any scribe vendor:
- Is a signed BAA included, at what tier, and who signs it?
- When is the audio recording deleted? After note generation, after 30 days, or on a user-controlled schedule?
- Is patient data used to train models? If so, can you opt out?
- Is there an on-device option that avoids cloud transmission entirely?
Freed deletes audio recordings automatically after the note is generated and states that protected health information is not used for AI training 7Ref 7Freed (getfreed.ai) (2026).Freed Privacy and Security (HIPAA).Freed deletes audio after note generation; PHI not used for AI training; BAA included on all paid plans. A signed BAA is included on all paid plans.
Gale's on-device iOS path keeps audio on the device and deletes it after transcription without cloud transmission. The web path transmits over an encrypted connection and deletes the recording post-note. PHI is not retained in the signals Gale uses to improve note quality.
Where to Use a Standalone Scribe vs. an Integrated OS
Standalone AI scribes make sense when:
- You are already fully configured with an EHR, billing, and scheduling you want to keep
- You need immediate access and are not evaluating a full platform change
- You want to test AI-assisted documentation with low switching cost
An integrated practice OS — one where scribe, EHR, billing, and scheduling share a data model — offers advantages that standalone scribes cannot:
- The note the scribe drafts flows into the same record that generates the claim; there is no copy-paste handoff
- Billing codes can be suggested from the note without a separate coding tool
- Credentialing status, payer enrollment, and the clinical record live in one system
The main downside of an integrated approach is that switching costs are higher if you later want to change platforms. Evaluate the full OS — not just the scribe — before committing.
Common questions
Is there a genuinely free AI medical scribe?
As of June 2026, no standalone AI scribe provides unlimited, fully HIPAA-compliant notes at zero cost indefinitely. Freed's free trial runs 7 days. Heidi Health's free tier caps at 10 Pro Actions per month. General-purpose AI tools (ChatGPT, Claude) have no HIPAA BAA for standard accounts and cannot legally process identifiable patient audio. Gale bundles the Jefferson scribe at $0/month as part of the practice OS — there is no separate scribe subscription — but Gale is currently in a pre-commercial demonstration phase.
How much time does an AI scribe actually save?
The most rigorous published data — a 2026 JAMA study of 8,581 clinicians across five health systems — found average documentation time fell by 16 minutes per 8-hour shift, and total EHR time fell by 13.4 minutes. Providers who used the scribe for at least half their visits saved roughly double that. After-hours documentation time did not show significant change at average adoption levels.
What is the difference between an ambient AI scribe and a traditional medical transcriptionist?
A traditional medical transcriptionist listens to a dictated recording (usually after the visit) and types a note. An ambient AI scribe listens passively during the live encounter without interrupting the conversation, and drafts the note automatically within minutes of visit end. Ambient AI scribes typically cost far less than human medical transcriptionists (often $0.10–$0.25 per minute of audio) and return a draft faster.
Does Gale's AI scribe require a subscription?
No. The Jefferson scribe is bundled in Gale's practice OS at no separate monthly fee. Gale earns a percentage of billing collections only on claims that actually pay — the software itself, including the scribe, is free to use. Gale is currently in a pre-commercial, synthetic-demonstration phase; no real patient billing is processed yet.
Is the audio recording from a scribe session stored or used to train AI?
It depends on the vendor. Freed deletes audio after note generation and does not use PHI for model training. Gale's on-device iOS option never transmits audio to the cloud. Gale's web-based path transmits audio over an encrypted connection and deletes it after transcription. In both cases, Gale's model improvement signals are de-identified — no patient identity is retained. Always confirm the specific retention policy and BAA terms with any vendor before use.
Does the provider have to review and approve the AI-generated note?
Yes — with Gale's Jefferson scribe, the AI produces a draft. The provider reviews, edits, and signs the note before it enters the record. The scribe is an efficiency tool, not an autonomous documentation system. The provider's edited and signed version is the clinical record of record.
How does Nuance DAX price for independent practices?
Nuance DAX Copilot (rebranded Dragon Copilot as of March 2025) is not designed for solo or small-group independent practices. Published estimates place list pricing at $369–$600 per provider per month, with one-time implementation fees of $5,000–$50,000 and minimum seat requirements that typically exclude practices with fewer than 10 providers. DAX is primarily an enterprise health system product.
