What Happens on the First Home Health Visit
SaveFamilies expect a nurse to arrive and start nursing. What arrives is a clipboard. The start-of-care visit is the most bureaucratic of the whole admission, and that is by design: the answers the nurse records set your visit frequency, decide what Medicare pays the agency, and end up in the agency's public quality rating. Who comes, and what the visit is actually for
A registered nurse, in most admissions, doing an admission rather than a treatment. The visit exists to answer three questions on Medicare's behalf: does this person qualify, what is actually going on in this house, and what will the agency do about it. Everything the nurse asks feeds one of those three. The therapy, the wound care, the teaching — that mostly starts on the visits after this one.
If the order is therapy-only, a physical or occupational therapist may do the admission instead, and the visit looks much the same. Either way it follows a home health referral: a doctor or allowed provider ordered the care, and somebody has to turn that order into a plan.
The tone catches families off guard. Someone has just come home from a hospital, frightened and exhausted, and a nurse spends the first hour with a laptop. It reads as indifference. It is not. The face-to-face encounter is the requirement that trips admissions up, because it happens before the nurse ever arrives and most families have no idea it exists. If the doctor did not document it in the right window, the agency will chase it — and the chase can delay a start of care everyone has already agreed to.

What to have ready before the nurse arrives
Every medication in the house, in its own bottle, including the ones nobody takes anymore. The visit goes faster when the nurse can see the actual containers instead of a list. Also worth putting on the table: the hospital discharge papers, the insurance and Medicare cards, the name and contact details of the doctor who ordered the care, and any advance directive or healthcare proxy that exists.
One more thing, and it is the one families skip: someone who knows the history should be in the room. The assessment asks about memory, mood, falls, and what a person can do alone — and the person being assessed is often the least reliable witness to all four, because the answer to how are you managing is almost always fine.

How the medication review works
