It has been in use since 1974, when it beat the alternatives in a head-to-head comparison of ways to measure pain. Half a century on, the line is still the simplest instrument in the room and still the most over-read: the mark measures one dimension of an experience with several, and it means far less across two people than it does across two Tuesdays.

What the visual analog scale is

It is a single question asked with a ruler instead of words. The person marks a line at the point representing their own pain, and the position of that mark — conventionally measured in millimetres from the no-pain end of a 100 mm line — becomes the score. Higher means worse. There is one item and nothing else in the instrument.

Its standing dates to a 1974 paper in the Lancet by Huskisson, which set the visual analog scale against verbal rating scales and pain-relief scales and concluded it was the most sensitive of the methods tested 1. That comparison is why it displaced the alternatives, and why it remains the reference method half a century later.

Visual analog means the answer is given as a position in space rather than as a word or a digit — the length of the line stands in for the size of the sensation.

The design has one real advantage over asking for a number out of ten. A line has no steps in it, so the small movements a person can feel but cannot name still register. Exact line length and the wording at each end vary between forms, which is a reason to read the form rather than assume it.

The line and the 0-to-10 question are not the same instrument

In everyday clinical use the two get merged, and the difference is worth keeping. The numeric pain rating scale asks for a spoken or written whole number: a single item scored 0 to 10, where 0 is no pain and 10 the worst imaginable pain, running in the same direction as the analog line so higher means worse and a falling score means improvement 2.

The practical distinction is granularity and format. The analog line is continuous and needs paper, a screen, or something to mark with; the numeric scale has eleven possible answers and can be asked over the phone or repeated hourly on a ward with no equipment at all. That convenience is why most people who think they were given a visual analog scale were actually given the numeric one.

A mark on a line and a spoken number out of ten are close relatives, not the same measure — and the published thresholds belong to whichever one was actually used.

The two correlate closely enough that clinicians often treat them interchangeably, and for tracking one person over a few weeks that is usually harmless. It stops being harmless when a threshold from one is quoted as though it applied to the other.

How much change counts as a real change

For the numeric scale, the answer has been worked out and it is smaller than most people guess. In a group of patients with low back pain receiving physical therapy, the minimal clinically important difference was 2.2 points at one week and 1.5 points at four weeks, while the standard error of measurement was 1.02, giving a minimum detectable change of about 2 points 2.

A change of about 2 points on the 0-to-10 scale is the defensible threshold — it is both clinically meaningful and larger than the measurement error 2. The four-week figure of 1.5 points falls below that error, which is why 2 points rather than 1.5 is the number worth holding on to.

Two caveats travel with those figures. They were derived in an outpatient population with low back pain and should not be assumed to transfer to other kinds of pain 2. And they belong to the numeric scale: the 1974 paper establishing the visual analog scale reports no minimal clinically important difference and no minimal detectable change of its own 1.

There is a quieter finding in that original paper that has never quite caught on. Huskisson concluded that for assessing response to treatment, a scale measuring pain relief has advantages over a scale measuring pain 1. Asking how much better it is turns out to be a different question from asking how bad it is, and often a more answerable one.

There is no mild, moderate or severe on this scale

The bands people assume exist are not part of the instrument. The validation work on the numeric scale defines no severity cutoffs — no threshold at which a score becomes moderate, no line above which pain is officially severe 2. Whatever chart divides the scale into coloured thirds, that division came from somewhere other than the measure.

This matters for comparison. The scale is anchored at one end by each person's own worst imaginable pain — a private reference point built from their own history. Two people who both mark a 7 have not reported the same sensation; they have reported the same fraction of two different scales.

Your number is not a claim you have to defend against anyone else's. It is a reading on your own instrument, and its usefulness is in how it moves.

What the scale does well is track one person over time, which is the job it is usually doing when it appears in a course of treatment for knee pain, hip pain or a bad back. What it does poorly is rank people against each other — asking the line to settle who is believed or who is treated first is asking for a comparison it was never calibrated to make.

The scale hidden inside other scores

Composite outcome measures often contain an analog pain line as a load-bearing component, which surprises people who assume a shoulder or knee score is built entirely from questions about activities. The ASES shoulder score is the clearest example: its patient section combines a visual analog scale for pain with a ten-item activities-of-daily-living questionnaire, and the pain scale supplies fully half of the resulting 0-to-100 score, in which higher indicates better function and less pain 3.

Half the score, from one mark on one line. That is not a criticism of the design — pain is a reasonable candidate for half the weight in a shoulder measure — but it does mean a shoulder score can move substantially on a day when nothing about the shoulder's function changed.

The same architecture recurs across musculoskeletal outcome measures, and it explains much of how they behave: a composite containing a pain line responds quickly to anything that eases pain and more slowly to changes in what a person can do.

What a pain number leaves out

Intensity is one dimension of pain, and on the evidence it is not the dimension that best predicts what happens next. Two other instruments, both developed in the 1990s, exist specifically because the number on the line was leaving so much unmeasured.

The pain catastrophizing scale is a 13-item self-report measure, each item rated 0 to 4 for a total of 0 to 52, higher scores meaning more catastrophizing; factor analysis produced its three enduring components of rumination, magnification and helplessness 4. High scorers report more negative pain-related thoughts, greater emotional distress and greater pain intensity under a controlled cold-pressor procedure 4.

The tampa scale of kinesiophobia measures something adjacent and equally invisible on a pain line: fear that movement will cause injury or re-injury, with higher scores meaning more fear. In its foundational study, those scores tracked catastrophizing and depression more strongly than pain intensity, and predicted fear and escape-avoidance behaviour on actual exposure to movement 5.

How much someone hurts and how much they avoid are different quantities, and the second is often the one that shapes the year ahead 5.

None of this makes the pain number useless. It makes it partial. A clinician who asks about the number and about what you have stopped doing is collecting two signals, and the second rarely appears on any line.

When the line does not work

The visual analog scale asks a person to map an internal sensation onto a spatial abstraction, and that request fails for more people than its ubiquity suggests. Young children, people with dementia or delirium, people with significant visual or motor impairment, and anyone being asked in a language they do not read fluently may produce a mark that means nothing — or none at all — while the chart records it as data.

The response is not to keep asking louder. Health literacy universal precautions means structuring communication so every patient can understand and be understood regardless of literacy level, using plain language and techniques such as teach-back to confirm the message landed 6. Applied to a pain scale: check the question was understood before recording the answer, and reach for a different instrument when it was not.

Alternatives exist for exactly these situations: verbal descriptor lists, faces scales for children, and observational measures scored from behaviour. Each measures something slightly different, and swapping between them mid-treatment breaks the comparison the number was collected for.

Not being able to put a number on your pain is a limitation of the instrument, not a failure of the patient. A clinician has other ways to ask, and describing what the pain stops you doing is often more informative than any score.

Common questions

Not exactly. The analog scale is a mark on a continuous line, measured with a ruler; rating out of ten is the numeric pain rating scale, with eleven possible answers. They measure the same construct and correlate well, but published thresholds for meaningful change belong to whichever version was actually used.

On the 0-to-10 numeric scale, a change of about two points is the usual reference, because that is both clinically meaningful and bigger than the measurement error of the scale itself. Smaller shifts can be real, but they cannot be distinguished from the ordinary noise of asking the same question twice.

Not officially. The scale defines no cutoffs separating mild from moderate from severe, and the anchors at each end are personal — the top of the line is whatever the worst imaginable pain means to that individual. The number's value lies in tracking one person over time rather than in grading anyone.

Because the scale is a snapshot of a moment, and the moment includes sleep, mood, distraction and what the day has demanded. That variability is one reason a single reading carries limited weight, and why clinicians look at a series of scores rather than treating any one of them as the answer.

Honest is better than consistent. The most useful thing a pain score can do is reflect what the day was actually like, and adjusting an answer to fit the previous one removes the signal. If the scale feels wrong for your pain, saying so gives a clinician more to work with than a compromised number does.

Not better — different. Function questionnaires such as the quebec back pain scale ask what a person can do rather than how much it hurts, and separate instruments measure catastrophizing and fear of movement. Used together they describe an experience that no single number captures, which is why clinics often hand out more than one form.

Pain that needs attention regardless of the number

  • Pain that reaches its worst intensity within seconds of starting — a sudden thunderclap headache, or abrupt tearing chest or back pain
  • Chest pressure or pain with breathlessness, sweating, nausea, or pain spreading into the jaw, neck or arm
  • Severe abdominal pain with a rigid or board-like abdomen, or with persistent vomiting and an inability to pass stool or gas
  • Pain in an arm or leg that is escalating out of proportion after an injury, cast or surgery, especially with numbness, tightness, or a cold or pale limb

Sudden worst-ever pain, chest pain with breathlessness or sweating, or a limb that is cold, pale or rapidly worsening after injury are emergencies. Call 911 rather than waiting to see whether the score comes down.
