Viagra (Sildenafil) Side Effects: What to Expect, What to Watch For, and When to Seek Help
Common sildenafil (Viagra) side effects are headache, facial flushing, and nasal congestion — usually mild and temporary. Sildenafil must never be combined with nitrate medications or recreational poppers; together they can cause a dangerous, potentially fatal drop in blood pressure. Review your full medication list with a clinician first.

Find care →Why does sildenafil cause these particular side effects?
Sildenafil belongs to a class called PDE5 (phosphodiesterase type 5) inhibitors. It works by blocking an enzyme that breaks down cGMP — a molecule that triggers smooth-muscle relaxation in blood vessel walls 1Ref 1Pyrgidis N, Mykoniatis I, Haidich AB, et al. (2021).The Effect of Phosphodiesterase-type 5 Inhibitors on Erectile Function: An Overview of Systematic Reviews.PDE5 inhibitor mechanism of action explaining common side effects (flushing, headache, nasal congestion, blue vision), delivery and absorption considerations, and alpha-blocker interaction. That relaxation allows blood to fill the erectile tissue of the penis during arousal, but the same mechanism acts throughout the body.
Widened blood vessels in the face cause flushing; in the sinuses, nasal congestion; in the skull, headache. The more serious effects emerge when sildenafil's blood-pressure-lowering action combines with other drugs that lower pressure through the same or amplifying mechanisms.
What are the common, expected side effects?
The most frequently reported effects are mild and self-limited 1Ref 1Pyrgidis N, Mykoniatis I, Haidich AB, et al. (2021).The Effect of Phosphodiesterase-type 5 Inhibitors on Erectile Function: An Overview of Systematic Reviews.PDE5 inhibitor mechanism of action explaining common side effects (flushing, headache, nasal congestion, blue vision), delivery and absorption considerations, and alpha-blocker interaction:
- Headache — the most common, typically peaking within an hour and resolving as the drug clears
- Flushing — warmth or redness in the face and neck
- Nasal congestion or runny nose
- Indigestion or upset stomach
- Blue-tinted or altered color vision — a benign transient effect from sildenafil's activity against PDE6, a related enzyme in the retina; distinct from sudden vision loss and usually resolves promptly
If these effects are severe, unusually prolonged, or accompanied by any of the warning signs listed below, contact a clinician.
What is the nitrate interaction and why is it a hard stop?
Nitrate medications — nitroglycerin tablets or spray, isosorbide mononitrate, isosorbide dinitrate — are prescribed for angina (heart-related chest pain). Both nitrates and sildenafil lower blood pressure through different but additive pathways. Together, the drop in blood pressure can be severe enough to cause fainting, heart attack, or stroke 2Ref 2Burnett AL, Nehra A, Breau RH, et al. (2018).Erectile Dysfunction: AUA Guideline.Nitrate contraindication with PDE5 inhibitors, cardiovascular risk assessment before prescribing ED medication, and ED as a marker of cardiovascular disease.
This is not a dose-dependent effect with a safe threshold — it is an absolute contraindication: if you take nitrates in any form, you cannot safely use sildenafil. Similarly, recreational amyl or butyl nitrite ("poppers") carry exactly the same dangerous interaction. If you take sildenafil and then develop chest pain, the emergency team must know this before any nitroglycerine is given — disclose it immediately 2Ref 2Burnett AL, Nehra A, Breau RH, et al. (2018).Erectile Dysfunction: AUA Guideline.Nitrate contraindication with PDE5 inhibitors, cardiovascular risk assessment before prescribing ED medication, and ED as a marker of cardiovascular disease.
Who should be especially careful before using sildenafil?
Men with known heart disease, a recent heart attack or stroke, uncontrolled high blood pressure, or very low blood pressure require a careful cardiovascular review before using any PDE5 inhibitor 2Ref 2Burnett AL, Nehra A, Breau RH, et al. (2018).Erectile Dysfunction: AUA Guideline.Nitrate contraindication with PDE5 inhibitors, cardiovascular risk assessment before prescribing ED medication, and ED as a marker of cardiovascular disease. Sexual activity itself places cardiovascular demand, independent of the medication.
Sildenafil is metabolized by the liver enzyme CYP3A4. Certain drugs — including HIV protease inhibitors, some antifungals, and CYP3A4 inhibitors — can markedly raise sildenafil blood levels, increasing both efficacy and side-effect risk 1Ref 1Pyrgidis N, Mykoniatis I, Haidich AB, et al. (2021).The Effect of Phosphodiesterase-type 5 Inhibitors on Erectile Function: An Overview of Systematic Reviews.PDE5 inhibitor mechanism of action explaining common side effects (flushing, headache, nasal congestion, blue vision), delivery and absorption considerations, and alpha-blocker interaction. Alpha-blockers used for blood pressure or prostate symptoms also lower blood pressure and can interact; dose management and timing coordination are required 1Ref 1Pyrgidis N, Mykoniatis I, Haidich AB, et al. (2021).The Effect of Phosphodiesterase-type 5 Inhibitors on Erectile Function: An Overview of Systematic Reviews.PDE5 inhibitor mechanism of action explaining common side effects (flushing, headache, nasal congestion, blue vision), delivery and absorption considerations, and alpha-blocker interaction.
A diagnosis of retinitis pigmentosa warrants extra caution given the mechanism by which sildenafil affects retinal enzymes. ED is often an early marker of cardiovascular disease or metabolic conditions — a prescribing visit is also an opportunity to assess overall heart health and screen for diabetes 2Ref 2Burnett AL, Nehra A, Breau RH, et al. (2018).Erectile Dysfunction: AUA Guideline.Nitrate contraindication with PDE5 inhibitors, cardiovascular risk assessment before prescribing ED medication, and ED as a marker of cardiovascular disease.
What affects how well sildenafil works?
Sildenafil is typically taken 30 to 60 minutes before sexual activity, but a heavy high-fat meal slows its absorption and blunts the peak effect — a light or empty stomach is preferred 1Ref 1Pyrgidis N, Mykoniatis I, Haidich AB, et al. (2021).The Effect of Phosphodiesterase-type 5 Inhibitors on Erectile Function: An Overview of Systematic Reviews.PDE5 inhibitor mechanism of action explaining common side effects (flushing, headache, nasal congestion, blue vision), delivery and absorption considerations, and alpha-blocker interaction. Alcohol independently worsens erectile dysfunction and amplifies the blood-pressure-lowering effect of sildenafil — not a recommended combination. Sexual stimulation is still required for sildenafil to work: the drug facilitates the response, it does not create it independently.
The right starting dose and what to do if it does not work
Sildenafil is available in 25 mg, 50 mg, and 100 mg tablets. Most clinicians start at 50 mg and adjust based on efficacy and tolerability. Starting at the lowest effective dose minimizes side effects without sacrificing therapeutic benefit 1Ref 1Pyrgidis N, Mykoniatis I, Haidich AB, et al. (2021).The Effect of Phosphodiesterase-type 5 Inhibitors on Erectile Function: An Overview of Systematic Reviews.PDE5 inhibitor mechanism of action explaining common side effects (flushing, headache, nasal congestion, blue vision), delivery and absorption considerations, and alpha-blocker interaction.
If sildenafil does not work on the first attempt, this does not always mean the medication has failed. Common reasons for suboptimal initial response include: taking the medication too close to a heavy meal, insufficient time before sexual activity, inadequate sexual stimulation, or significant anxiety 1Ref 1Pyrgidis N, Mykoniatis I, Haidich AB, et al. (2021).The Effect of Phosphodiesterase-type 5 Inhibitors on Erectile Function: An Overview of Systematic Reviews.PDE5 inhibitor mechanism of action explaining common side effects (flushing, headache, nasal congestion, blue vision), delivery and absorption considerations, and alpha-blocker interaction. Clinicians typically recommend trying the medication several times across different conditions before concluding it is ineffective.
If dose adjustment does not help or side effects are limiting, switching to tadalafil or another PDE5 inhibitor is a reasonable next step — there is meaningful individual variation in response across the drug class. A clinician can guide the choice based on your lifestyle preferences and health history 2Ref 2Burnett AL, Nehra A, Breau RH, et al. (2018).Erectile Dysfunction: AUA Guideline.Nitrate contraindication with PDE5 inhibitors, cardiovascular risk assessment before prescribing ED medication, and ED as a marker of cardiovascular disease.
Common questions
Can I take sildenafil if I have high blood pressure?
Possibly — controlled high blood pressure is not by itself a contraindication. But blood pressure medications interact differently with sildenafil, particularly alpha-blockers and nitrates. A clinician will review your specific regimen and blood pressure control before recommending it.
How long does an erection from sildenafil last?
Sildenafil produces an effect window of roughly four to six hours, but does not cause an erection by itself — it requires sexual arousal. An erection lasting more than four hours without arousal (priapism) is a medical emergency requiring immediate attention.
Is it safe to drink alcohol while taking sildenafil?
Moderate alcohol use is unlikely to cause a serious interaction, but alcohol independently worsens erectile dysfunction and can amplify sildenafil's blood-pressure-lowering effect. Heavy alcohol use before or during use is not recommended.
Are there other ED medications similar to Viagra with different side-effect profiles?
Yes — tadalafil (Cialis), vardenafil (Levitra), and avanafil (Stendra) are in the same drug class. They share the nitrate contraindication but differ in onset, duration, and food interaction profile. A clinician can help determine which suits your situation and health history best.
Can counterfeit Viagra be dangerous?
Yes. Counterfeit or unregulated ED medications purchased without a prescription may contain incorrect doses, undisclosed active ingredients, or harmful contaminants. Only use medications obtained through a licensed clinician and a legitimate pharmacy.
