What is the Palliative Performance Scale?

The Palliative Performance Scale, or PPS, is a bedside tool that scores function from 100% — fully active — down to 0%, in steps of ten, by reading five observable things at once: ambulation, activity level and evidence of disease, self-care, oral intake, and level of consciousness 1. It is a modification of the older karnofsky score, adapted for people approaching the end of life, where eating and wakefulness carry information the original scale never captured 1.

The appeal is speed and honesty. No lab draw, no questionnaire: a nurse who has watched someone move from bed to chair and take lunch can assign a PPS in minutes, and repeat it next week to see which way things are moving. That repeatability is what makes it a workhorse in hospice and palliative charts.

How is a PPS score assigned?

The scale is a grid: rows from 100% down to 0%, five columns across. A scorer reads across and picks the row that best fits the person in front of them, with the leftmost columns — ambulation and activity — generally leading the determination 1. Someone who spends most of the day in bed cannot score above the rows that say so, however good their appetite happens to be.

A family reading a chart entry like "PPS 40%" can translate it roughly as: in bed or a chair most of the day, needing a great deal of help. The more useful question for the clinician who wrote it is which column moved since last time — walking, eating, or wakefulness — because those tell different stories.

How does the PPS figure into hospice eligibility?

Medicare hospice eligibility rests on a physician's certification that life expectancy is six months or less, and the Local Coverage Determination contractors use asks for that judgment to be supported with documented decline — functional, nutritional, or both 2. A PPS recorded at successive visits is one of the cleanest ways to show it: 60% in March, 50% in May, 40% in June tells a reviewer what pages of narrative cannot. The same guidance is explicit that its numbers are guidance in support of a prognosis, not absolute cutoffs — so no single PPS value qualifies or disqualifies anyone 2.

The scale appears across diagnoses. It figures into hospice eligibility for cancer, where performance decline is often the loudest signal, and into the same framework's guidance for other conditions — including hospice eligibility after stroke. Families can also track the observable pieces themselves between visits — how much of the day is spent lying down, how meals are going, how long the awake hours last — and bring that record in. A clinician cannot visit every week; a caregiver sees every day. When the two sets of observations line up, the conversation about what comes next tends to happen earlier, with less guesswork and fewer surprises.

When a change in function needs a call now
