Several frameworks have expanded what is possible:
Interstate Medical Licensure Compact (IMLC). Nurse Licensure Compact (NLC). For RNs and NPs, the NLC allows a single multistate license that explicitly covers telehealth services to patients located in any of the 43 participating jurisdictions 2Ref 2National Council of State Boards of Nursing (2024).Nurse Licensure Compact Commission 2024 Annual Report.NLC multistate license explicitly covers telehealth services; 43 jurisdictions participate as of fiscal year 2024; RNs and NPs may provide care across state lines without additional licenses. PSYPACT and Counseling Compact. Psychologists in 40+ participating states can provide telepsychology under PSYPACT. The Counseling Compact covers licensed counselors in member states.
Practices with broad licensure. Large telehealth platforms and multi-state practices deliberately license their clinicians in many states. Ask your practice which states their clinicians are licensed in — they may cover where you are traveling.
Limited episodic-care exceptions. Are controlled substance prescriptions even more restricted?
Yes. If your visit involves a controlled substance prescription — certain ADHD medications, anxiety medications, sleep aids, or pain medications — the rules are stricter. Prescribing controlled substances across state lines carries additional federal and state requirements beyond the licensure question. If you need a refill of a controlled substance while traveling, contact your home practice before you leave to plan ahead.
What should you do before a visit if you will be out of state?
1. Tell your practice ahead of time. A simple portal message — "I will be in [state] on the day of my appointment, does this affect anything?" — gives them time to check licensure and reschedule if needed. 2. Ask if they can see you. Some practices have colleagues licensed in your destination state. 3. Consider rescheduling. If your concern is not urgent, rescheduling until you are home is often the cleanest option. 4. For urgent non-emergency needs while traveling: A national telehealth platform (many are licensed across most or all states) or an urgent care clinic in your destination city are practical alternatives. 5. For true emergencies: Your location does not affect emergency care — go to the nearest emergency department or call 911.
Does insurance coverage change when you are out of state?
Possibly. Even if the licensure issue is resolved, your insurance plan's in-network coverage may differ in another state. Many plans cover emergency care regardless of location but apply different cost-sharing or network rules for non-emergency visits. Call your insurer before the visit to confirm coverage — especially if the visit is not through a platform you use regularly at home.

What is an interstate medical licensing compact?
It is an agreement among participating states that allows a clinician licensed in one member state to practice in other member states without obtaining a separate full license in each. Can my therapist see me over video while I am traveling out of state?
Only if they hold a license — or a compact license — in the state where you physically are. PSYPACT covers 40+ states for psychologists. The Counseling Compact covers some states for licensed counselors. Check with your therapist before your trip.
What if I just need a routine prescription refill while traveling?
Contact your home practice through the portal or by phone before you leave. For non-controlled medications, they may be able to send a refill to a pharmacy in your travel state. Controlled substances are more restricted — plan ahead.
