Sildenafil vs Tadalafil for ED: Differences, Duration, Side Effects
Sildenafil (Viagra) and tadalafil (Cialis) are both PDE5 inhibitors that treat erectile dysfunction. The key difference is duration: sildenafil works for 4–6 hours; tadalafil lasts up to 36 hours. Tadalafil is also available as a low-dose daily option. Both require sexual stimulation to work.
How do sildenafil and tadalafil work?

What are the common side effects?

- Visual disturbances: Sildenafil can cause a temporary blue-tinted or brightened visual effect; this is less common with tadalafil, which has less cross-reactivity with PDE6 in the retina

Most side effects are mild and transient. What other ED options exist?
Two other PDE5 inhibitors — vardenafil (Levitra) and avanafil (Stendra) — work similarly and are alternatives if sildenafil or tadalafil is not well tolerated. Avanafil has a faster onset (15–30 minutes) and may have fewer visual side effects.
For men who do not respond to oral PDE5 inhibitors, or who have contraindications, alternatives include: - Penile injections (alprostadil) - Urethral suppositories (alprostadil intraurethral pellets) - Vacuum erection devices - Penile implant surgery (for refractory cases)

Common questions
Do I need to take sildenafil every day, or just before sex?
Sildenafil is typically taken on-demand, 30–60 minutes before anticipated sexual activity. It is not intended for daily dosing at the standard prescription doses (25 mg, 50 mg, 100 mg). Tadalafil is the medication available in a daily low-dose formulation.
Can I drink alcohol with these medications?
Moderate alcohol is generally acceptable, but alcohol is a vasodilator — it lowers blood pressure on its own. Combining alcohol with a PDE5 inhibitor can potentiate blood pressure lowering, increasing the risk of dizziness or fainting. Heavy drinking before using these medications is not advisable.
Why can't I take these if I use nitrate medications?
Nitrate medications (such as nitroglycerin for chest pain, or isosorbide mononitrate) also lower blood pressure. Combined with a PDE5 inhibitor, the drop in blood pressure can be dangerously severe and can cause syncope or cardiovascular collapse. This combination is absolutely contraindicated.
Is generic sildenafil the same as Viagra?
Yes. Generic sildenafil contains the same active ingredient as Viagra. It became available after patent expiration and is substantially cheaper while being therapeutically equivalent.
Sildenafil isn't working for me. What should I do?
Check that you are taking it at the right dose, at the right time (30–60 minutes before, on a light stomach), and with adequate sexual stimulation. If it is truly not working at an adequate dose, your clinician may switch you to tadalafil, evaluate for underlying causes like low testosterone, or refer you to a urologist.
Safety: who should not take these medications
- —Do NOT combine with any nitrate medication (nitroglycerin, isosorbide) — the blood pressure drop can be life-threatening
- —Men with severe heart disease, recent heart attack or stroke, or very low blood pressure should discuss cardiac safety with a physician before using PDE5 inhibitors
- —Sudden vision loss or sudden hearing loss after taking a PDE5 inhibitor requires immediate medical attention
- —Prolonged erection lasting more than 4 hours (priapism) is a medical emergency — go to an emergency department immediately
Priapism (erection lasting over 4 hours) requires emergency treatment to prevent permanent damage. Sudden vision loss or chest pain after taking these medications requires calling 911.
