PCOS Hirsutism: Facial Hair Treatment Options
Hirsutism in PCOS — coarse, dark facial or body hair — results from elevated androgen levels. Treatment combines two tracks: medications such as spironolactone or oral contraceptives to slow new hair growth, and cosmetic methods like laser hair removal or eflornithine cream to address existing hair.
What causes facial hair in PCOS?
Polycystic ovary syndrome raises androgen levels — hormones such as testosterone and DHEA-S that stimulate hair follicles. When androgen activity in follicles is elevated, fine vellus hair can convert to coarser, darker terminal hair in areas typically associated with male-pattern growth: the upper lip, chin, jaw, chest, lower abdomen, and inner thighs.
Hirsutism is one of the most distressing symptoms reported by people with PCOS 1Ref 1Teede HJ, Tay CT, Laven JJE, Dokras A, et al. (2023).Recommendations From the 2023 International Evidence-based Guideline for the Assessment and Management of Polycystic Ovary Syndrome.Hirsutism is a common and distressing symptom of PCOS driven by androgen excess; first-line treatment includes combined hormonal contraceptives. It does not go away on its own without addressing the underlying hormonal imbalance, which is why a clinician's evaluation matters.
What medications help slow PCOS hair growth?
Hormonal contraceptives. Combined oral contraceptive pills (estrogen + progestin) are a first-line medical option. They lower androgen production from the ovaries and raise sex hormone-binding globulin (SHBG), which reduces the amount of free testosterone available to follicles. Clinical improvement typically appears over three to six months 1Ref 1Teede HJ, Tay CT, Laven JJE, Dokras A, et al. Spironolactone. This medication was originally developed for blood pressure but acts as an androgen-receptor blocker. It is commonly added when contraceptives alone do not fully control hirsutism. Spironolactone requires reliable contraception in people who could become pregnant because of potential effects on a male fetus. Your clinician will weigh this carefully 2Ref 2Legro RS, Arslanian SA, Ehrmann DA, Hoeger KM, Murad MH, Pasquali R, Welt CK (2013).Diagnosis and treatment of polycystic ovary syndrome: an Endocrine Society clinical practice guideline.Spironolactone and combined oral contraceptives are recommended medical treatments for hirsutism in PCOS; metformin has modest androgen-lowering effects.
Eflornithine cream. Applied directly to facial skin, eflornithine (Vaniqa) slows the growth of new hair without removing existing hair. It is used alongside hair removal rather than instead of it.
Metformin. While primarily used for insulin resistance in PCOS, metformin can modestly lower androgen levels over time. Its effect on hirsutism alone is smaller than that of anti-androgenic medications 2Ref 2Legro RS, Arslanian SA, Ehrmann DA, Hoeger KM, Murad MH, Pasquali R, Welt CK (2013).Diagnosis and treatment of polycystic ovary syndrome: an Endocrine Society clinical practice guideline.Spironolactone and combined oral contraceptives are recommended medical treatments for hirsutism in PCOS; metformin has modest androgen-lowering effects.
All medication options require months of consistent use before results become visible. Stopping medication often allows hair to return.
What hair removal methods work alongside medication?
Medication slows new growth; it does not erase hair already present. Most people combine a medical approach with a cosmetic one:
- Laser hair removal and intense pulsed light (IPL): These target the pigment in the hair shaft and damage the follicle. Multiple sessions are typically needed. Darker hair on lighter skin responds best. Results can be long-lasting but may require maintenance sessions.
- Electrolysis: The only FDA-recognized permanent hair removal method. A fine probe is inserted into each follicle and an electrical current destroys it. It is slower and more suitable for smaller areas.
- Shaving, waxing, threading, and depilatory creams: Temporary methods that are safe to use at any time alongside medical treatment.
Eflornithine (Vaniqa) cream applied directly to the face slows hair growth locally and is often used as a bridge while waiting for laser results or medication to take effect 1Ref 1Teede HJ, Tay CT, Laven JJE, Dokras A, et al. Which specialist should I see for PCOS hirsutism?
An endocrinologist or gynecologist with PCOS experience leads the hormonal evaluation and manages medications. A dermatologist is a good resource for hair removal guidance and topical treatments such as eflornithine. Gale can help you prepare for any of these appointments — organizing your symptoms, tracking menstrual cycles, and forming your questions.
Common questions
How long does spironolactone take to work for PCOS facial hair?
Most people notice a reduction in new hair growth after three to six months of consistent use. Full effects can take up to a year. Existing terminal hairs do not disappear with medication — a hair removal method addresses those.
Can I use laser hair removal while taking spironolactone?
Yes. Laser hair removal and spironolactone address different parts of the problem — one removes existing hair, the other slows regrowth. Many clinicians recommend combining them. Confirm timing and any skin-sensitivity considerations with your dermatologist.
Will hirsutism come back if I stop the pill?
In most cases, androgen activity returns to its pre-treatment level after stopping hormonal contraceptives, and hair growth can resume. This is why ongoing management, not a short course, is the typical approach in PCOS.
Is hirsutism the same as PCOS?
No. Hirsutism is one possible symptom of PCOS, but it can also occur in people without PCOS, and not everyone with PCOS develops visible excess hair. A proper evaluation identifies the underlying cause.
When to seek care
- —Rapid onset of significant new hair growth over weeks (not months) — this can indicate a more urgent hormonal cause such as a tumor and needs prompt evaluation
- —Deepening voice, significant clitoral enlargement, or other sudden virilizing changes alongside new hair growth
- —Hirsutism accompanied by signs of Cushing syndrome: rapid weight gain centered in the trunk, stretch marks, easy bruising
