Motion Sickness Medications: How They Work and How to Choose
Motion sickness medications work best when taken before symptoms begin, since nausea slows drug absorption [1]. OTC antihistamines (dimenhydrinate, meclizine) are the most common first choice for healthy adults; the prescription scopolamine patch suits longer journeys. The right option depends on trip length, travel mode, and health history.

Find care →Why does motion sickness happen?
Motion sickness occurs when the brain receives conflicting signals from the inner ear, eyes, and body. On a boat, for example, the vestibular system senses motion that the eyes — staring at a still cabin wall — do not confirm. This sensory mismatch triggers nausea, sweating, pallor, dizziness, and sometimes vomiting 1Ref 1Centers for Disease Control and Prevention (2024).Motion Sickness — Yellow Book.Overview of motion sickness mechanisms; age susceptibility (children 2–12 most susceptible, adults >50 less so); medication options including antihistamines and scopolamine; onset timing (antihistamines ~2 hrs, scopolamine patch 2–4 hrs); gastric stasis impairing oral drug absorption after nausea onset; non-pharmacological strategies. Children aged 2–12 are especially susceptible; adults over 50 are generally less so. There is genuine biological variation in susceptibility.
How do over-the-counter antihistamines compare?
Dimenhydrinate (Dramamine) and meclizine (Bonine, Antivert) are the two antihistamines most commonly used for motion sickness. They dampen vestibular signals that trigger nausea 1Ref 1Centers for Disease Control and Prevention (2024).Motion Sickness — Yellow Book.Overview of motion sickness mechanisms; age susceptibility (children 2–12 most susceptible, adults >50 less so); medication options including antihistamines and scopolamine; onset timing (antihistamines ~2 hrs, scopolamine patch 2–4 hrs); gastric stasis impairing oral drug absorption after nausea onset; non-pharmacological strategies.
- Dimenhydrinate acts faster, is available in children's formulations, and generally causes more sedation.
- Meclizine tends to cause less drowsiness and has a longer duration per dose — useful for longer trips.
Both must be taken well before travel begins — onset is typically about two hours, and once nausea is established, absorption is impaired 1Ref 1Centers for Disease Control and Prevention (2024).Motion Sickness — Yellow Book.Overview of motion sickness mechanisms; age susceptibility (children 2–12 most susceptible, adults >50 less so); medication options including antihistamines and scopolamine; onset timing (antihistamines ~2 hrs, scopolamine patch 2–4 hrs); gastric stasis impairing oral drug absorption after nausea onset; non-pharmacological strategies. Both cause drowsiness and interact with alcohol and other sedating medications. People who need to drive or operate machinery should use these with caution.
When is the prescription scopolamine patch the right choice?
Scopolamine (an anticholinergic) delivered via a transdermal patch placed behind the ear is effective for up to three days per patch, making it well suited for cruises, overnight ferries, and other multi-day motion-heavy journeys 1Ref 1Centers for Disease Control and Prevention (2024).Motion Sickness — Yellow Book.Overview of motion sickness mechanisms; age susceptibility (children 2–12 most susceptible, adults >50 less so); medication options including antihistamines and scopolamine; onset timing (antihistamines ~2 hrs, scopolamine patch 2–4 hrs); gastric stasis impairing oral drug absorption after nausea onset; non-pharmacological strategies. A Cochrane systematic review of 14 randomized controlled trials found scopolamine more effective than placebo and equivalent to antihistamines in preventing motion sickness symptoms 2Ref 2Spinks A, Wasiak J (2011).Scopolamine (hyoscine) for preventing and treating motion sickness.Systematic review of 14 RCTs (1,025 participants): scopolamine more effective than placebo and equivalent to antihistamines in preventing motion sickness symptoms.
Because it requires a prescription, a clinician visit is necessary. Side effects include dry mouth, blurred vision, and drowsiness. It is not appropriate for everyone: people with narrow-angle glaucoma or risk of urinary retention should not use it 1Ref 1Centers for Disease Control and Prevention (2024).Motion Sickness — Yellow Book.Overview of motion sickness mechanisms; age susceptibility (children 2–12 most susceptible, adults >50 less so); medication options including antihistamines and scopolamine; onset timing (antihistamines ~2 hrs, scopolamine patch 2–4 hrs); gastric stasis impairing oral drug absorption after nausea onset; non-pharmacological strategies. A trial dose before your trip is advisable to check tolerability before you are at sea.
What non-medication strategies actually help?
Behavioral and positioning strategies can supplement — or sometimes replace — medication 1Ref 1Centers for Disease Control and Prevention (2024).Motion Sickness — Yellow Book.Overview of motion sickness mechanisms; age susceptibility (children 2–12 most susceptible, adults >50 less so); medication options including antihistamines and scopolamine; onset timing (antihistamines ~2 hrs, scopolamine patch 2–4 hrs); gastric stasis impairing oral drug absorption after nausea onset; non-pharmacological strategies:
- Position: Front seat of a car, over the wing on a plane, center of a boat on the main deck — where motion is felt least.
- Visual horizon: Looking at a fixed point on the horizon reconciles the sensory mismatch that triggers nausea.
- Fresh air: Reduces nausea in many travelers.
- Avoid screens and books: Reading accelerates symptom onset in susceptible people.
- Avoid heavy meals and alcohol before and during travel.
- Acupressure wristbands (P6 point): evidence is mixed, but they carry no risk and may help some people.
These approaches are worth trying alongside medication rather than instead of it when susceptibility is high.
When should you talk to a pharmacist or clinician?
A pharmacist can help choose between over-the-counter options based on your other medications, trip duration, and need to stay alert. If OTC antihistamines have not worked in the past, or if you are planning a long sea voyage or have other health conditions, a clinician visit is the right step to discuss prescription options.
Older adults warrant special attention: antihistamines cause stronger sedation and increased fall risk in older adults, and scopolamine carries additional anticholinergic risks in this age group 1Ref 1Centers for Disease Control and Prevention (2024).Motion Sickness — Yellow Book.Overview of motion sickness mechanisms; age susceptibility (children 2–12 most susceptible, adults >50 less so); medication options including antihistamines and scopolamine; onset timing (antihistamines ~2 hrs, scopolamine patch 2–4 hrs); gastric stasis impairing oral drug absorption after nausea onset; non-pharmacological strategies. Clinician guidance is especially important here.
Common questions
Should I take motion sickness medication before or after I start feeling sick?
Before. Antihistamines and scopolamine work best when taken in advance. Once nausea is established, gastric stasis impairs absorption of oral medications, making them much less effective. Take an antihistamine at least an hour before boarding; apply the scopolamine patch several hours before travel.
Is Dramamine safe for children?
Dimenhydrinate is available in children's formulations, but dosing differs by age and weight, and some preparations are not appropriate for very young children. A pediatrician or pharmacist should guide selection and dosing.
Are there motion sickness options for older adults?
Antihistamines can cause stronger sedation and increased fall risk in older adults. Anticholinergic medications such as scopolamine carry additional risks including confusion in this age group. Clinician guidance is especially important — the right choice may differ significantly from what is appropriate for a younger person.
How does the scopolamine patch compare to antihistamines?
A Cochrane review found transdermal scopolamine more effective than placebo and roughly equivalent to antihistamines for preventing motion sickness. The patch has the advantage of three-day duration, which is useful for cruises, but requires a prescription, and side effects (dry mouth, blurred vision) affect some users.
