How Medicaid Pays for Home Care in Missouri
SaveTwo Missouri departments share one home care case: MO HealthNet decides whether a person has coverage, and Health and Senior Services decides whether they need care and how much. The threshold is a point score. The best-known program, Consumer-Directed Services, hands the hiring to the person receiving the care. Here is how the pieces connect, and where they jam.

MO HealthNet, and the two departments behind one case
Missouri splits a single home care case between two state departments, and knowing which is which saves months. MO HealthNet, inside the Department of Social Services, decides whether a person is financially eligible for Medicaid. The Division of Senior and Disability Services, inside the Department of Health and Senior Services, decides whether they need care, scores how much, and authorizes the hours.
Approval from one is not approval from the other. Two applications, two timelines, two sets of staff who do not share a desk. Families routinely spend a season waiting on the wrong one, and the phrase that unsticks it is usually a plain question: which department is my case sitting in right now?
The structure underneath matters too. Medicaid reaches home care through different authorities — some services sit in a state's regular benefit package, others in a waiver — and the choice is not cosmetic 1Ref 1Centers for Medicare & Medicaid Services (2025).Home & Community Based Services Authorities. That Medicaid covers home- and community-based services through distinct authorities — state plan options versus 1915(c) waivers — and that which authority a state uses is a structural choice with consequences for access.. Missouri put personal care in the state plan. A state plan benefit is not a fixed number of seats. That single fact separates Missouri from a good many of its neighbors.
Consumer-Directed Services: Missouri's biggest difference
Consumer-Directed Services is Missouri's flagship, and it inverts the usual arrangement. The participant is the employer. They recruit, hire, train, schedule, supervise, and if it comes to it, fire their own attendant, while a vendor handles payroll and withholding. Medicaid's self-direction rules exist for exactly this: a participant managing a budget and selecting, hiring, training, and managing their own caregiver 2Ref 2Centers for Medicare & Medicaid Services (2025).Self-Directed Services. That Medicaid self-direction lets a beneficiary manage a budget and select, hire, train, and manage their own caregivers, including a family member in some states — the federal basis for Missouri's Consumer-Directed Services model..
CDS is built for adults with physical disabilities who can direct their own care, or who have someone authorized to direct it for them. The attendant can be a neighbor, a friend, or in many cases an adult child. Age and relationship rules exist, they have moved over the years, and legally responsible individuals — a spouse, the parent of a minor — face tighter limits than other relatives.
There is a real cost to the control, and it is the part rarely said out loud. An agency carries a bench: when the aide calls in sick, someone else is dispatched. A CDS participant is the staffing department. If the attendant quits on a Tuesday, the participant is recruiting on Wednesday, from a hospital bed if that is where they happen to be.
The point score that decides everything
Missouri does not ask whether someone needs a lot of help. It scores them. The assessment run by the Division of Senior and Disability Services produces a number, and the nursing facility level of care sits at 21 points. Below that line the door is closed, regardless of how difficult the situation looks to the family standing in it.
The points come from concrete things: help needed with bathing, dressing, transfers, toileting, eating, mobility, medications, and how much supervision the person requires. It is worth understanding two things about the number.
- The threshold is a policy lever, not a clinical constant. Missouri has moved it before. Moving it changes who qualifies without anyone's health changing at all.
- Ask for the score itself, not just the outcome. Families are frequently told "denied" and never told the number. The number is the only thing that says whether the decision was close, and it is what an appeal argues about.
A denial carries formal appeal rights. So does an authorization that lands lower than expected, which is the more common and more quietly damaging result.
What MO HealthNet buys at home, and what it will not
Missouri covers personal care as a state plan benefit — help with bathing, dressing, transfers, toileting, and meals — so the everyday help is not automatically rationed behind a queue the way it is in states that buy it only through waivers. Waivers then sit on top, adding what the state plan does not: respite, adult day services, home modifications, and more, for people at a nursing facility level of care 3Ref 3Centers for Medicare & Medicaid Services (2025).Home & Community-Based Services 1915(c).That 1915(c) waivers add services such as respite, homemaker, and adult day care for people at an institutional level of care, must be cost-neutral against the institution they replace, and permit states to cap enrollment..
Missouri's Aged and Disabled Waiver is the main one for older adults; an Independent Living Waiver serves younger adults with physical disabilities. A waiver must cost no more than the institution it replaces, and federal law lets a state cap how many people it enrolls 3Ref 3Centers for Medicare & Medicaid Services (2025).Home & Community-Based Services 1915(c).That 1915(c) waivers add services such as respite, homemaker, and adult day care for people at an institutional level of care, must be cost-neutral against the institution they replace, and permit states to cap enrollment.. That is why the waiver layer behaves differently from the state plan layer underneath it.
None of this is Medicare. Medicare pays certified home health agencies through a prospective payment system for skilled, intermittent care 4Ref 4Centers for Medicare & Medicaid Services (2025).Home Health Prospective Payment System (Home Health PPS).That Medicare pays certified home health agencies through a prospective payment system — a distinct structure from the Medicaid-funded and private-pay home care this page describes. — nursing, therapy, a medical problem expected to change. It does not pay for the ongoing custodial help people mean by home care, which is paid out of pocket, by Medicaid for those who qualify, or by long-term care insurance 5Ref 5Administration for Community Living (2025).Costs of Care. That ongoing custodial and personal care at home is paid out of pocket, by Medicaid for those who qualify, or by long-term care insurance, because Medicare does not cover ongoing custodial care..
Missouri expanded Medicaid by ballot, and it took a court
Missouri voters approved Medicaid expansion at the ballot box in August 2020. The state did not implement it on schedule, litigation followed, and coverage began in 2021 after the Missouri Supreme Court ruled that the expansion had to proceed. This is not trivia. It is why some Missourians who are eligible today still believe they are not, having been told once that they were not.
Expansion widened the front door: a working-age adult under the income limit can hold a MO HealthNet card now without being found disabled first. In a non-expansion state that same person often has no route to Medicaid at any income.
But coverage is a prerequisite, not a service. Holding a MO HealthNet card does not by itself produce an attendant. The card satisfies the Department of Social Services. The hours still require the other department's assessment, the point score, and an authorization. People who clear the first gate and assume they are finished lose months at the second.
The spend down, when income sits too high
Missouri runs a spend down, which means income above the MO HealthNet limit does not end the conversation. The excess works like a monthly deductible: a person either incurs that much in medical expenses or pays it to the state, and coverage runs for the month once they have. No state is required to offer this, and not all of Missouri's neighbors do.
The rest of the financial test holds few surprises, and its shape is stable even though its numbers are not:
- Assets are counted separately from income, and clearing one test does nothing for the other.
- A home the person lives in, or intends to return to, is generally not a counted asset.
- Spousal impoverishment rules protect the spouse who stays in the community, so one person needing care does not require both to be impoverished.
The dollar figures change annually, which is why they are not printed here. The Department of Social Services publishes the current limits, and those are the only ones that count on the day of the application.
Where Missouri's home care actually breaks down
The Missouri failure mode is rarely a denial. It is an authorization that cannot be staffed: approved hours, a fee schedule that sets what an attendant can earn, and nobody willing to take the shift — most acutely in the rural counties between the two metros. Under Consumer-Directed Services this lands squarely on the participant, because the participant is the employer and there is no agency bench behind them.
The national backdrop is a rationing story. In 2025, 41 states reported waiting or interest lists for home and community based services, roughly 0.7 million people were on them, and the average wait for waiver services ran about 32 months 6Ref 6KFF (Kaiser Family Foundation) (2025).A Look at Waiting Lists for Medicaid Home- and Community-Based Services from 2016 to 2025.That 41 states reported HCBS waiting or interest lists in 2025, roughly 0.7 million people were on them, and the average wait for waiver services was about 32 months.. Missouri's state plan personal care does not queue the way a capped waiver does, which is a genuine structural advantage — but a benefit nobody will work is not care, and the state plan does not fix that.
The practical move is to treat recruiting as part of the application rather than something that begins after it. People who have a candidate in mind when the authorization lands start receiving care in weeks. People who start looking afterward often do not.
None of this survives a move. The general shape of medicaid home care is federal, and the answer to does medicaid pay for home care is yes nearly everywhere — but medicaid home care in Arkansas, medicaid home care in Colorado, and medicaid home care in California are different programs with different thresholds and different queues.

Say it back
How would you explain this to someone you love?
Two or three sentences, just as you’d say it. Gale reflects back what you focused on — a mirror, not a quiz.
The part that will not wait for an authorization
- —A fall the person cannot get up from, or one that leaves new pain, a bruise, or a cut, especially when nobody was present
- —New confusion, sudden drowsiness, or speech that has abruptly stopped making sense — a medical change, not a staffing problem
- —A reddened area over the tailbone, hips, or heels that does not fade once pressure is off it, or skin that has broken open
- —An attendant who has stopped coming, leaving someone who cannot transfer, toilet, or take their medications on their own
New confusion, chest pain, trouble breathing, a sudden change in alertness, or a fall someone cannot get up from is a 911 call, not a call to the case manager. The authorization will still be there afterward.
Gale's health library explains how public benefit programs are built. This is not legal, financial, or medical advice, and it is not an eligibility determination. Program rules, income limits, and level of care thresholds change; MO HealthNet and the Department of Health and Senior Services are the authority on your own case.
