Can you tell hemorrhoids from cancer by how the blood looks?

No — not reliably, and not from a photograph. Hemorrhoids and anal fissures are common, and most visible rectal bleeding does turn out to come from the anal canal rather than from anything malignant. But a colorectal cancer bleeds in the same colors as a hemorrhoid, and hemorrhoids are common enough that a great many people who have a polyp also have a hemorrhoid handy to blame it on.

That last point is the whole reason blood in stool and cancer stay tied together in every clinician's head. The presence of a benign explanation is not the absence of a serious one. Finding a hemorrhoid on an exam explains a hemorrhoid. It does not explain the blood, unless the two have been matched up deliberately — the bleeding site actually seen, the story actually fitting, the rest of the colon accounted for by age and risk.

Blood when wiping — red on the paper and nowhere else — is the pattern that generates the most searches and the least alarm, and in a person under 45 with no other symptoms it is usually treated the way it looks. Age is the single strongest thing you bring to this question, and it does more work than any description of the blood.
