Why does low iron point at the gut?

Iron deficiency anemia is, for most adults, a bleeding diagnosis wearing a nutritional costume. The body has no real route for dumping iron — it recycles what it has and holds on. Losing iron in quantity means losing blood, because most of the body's iron sits inside circulating red cells. So the useful question is not why the iron is low. It is where the blood is going.

That reframing is the whole reason this finding gets taken seriously. A person hears "you're anemic, start iron" and reasonably files it next to "you're low on vitamin D" — a deficiency, a supplement, done. But vitamin D goes low because you did not make or eat enough of it. Iron in an adult usually goes low because it left, and it left in blood.

In an adult, iron deficiency is a question about blood loss before it is a question about diet.

There are only a few places an adult loses blood in quantity without noticing. The uterus is one, and in someone who menstruates it is common enough to be the working answer. The gastrointestinal tract is the other. It runs from mouth to anus, its lining is thin and richly supplied with vessels, and — this is the part that matters — nothing about a small bleed there produces a sensation. There are no pain fibers in the gut lining that report slow oozing. Blood is simply digested on its way through, or diluted into stool in amounts too small to see.

The kidney and the airway can bleed too, but those announce themselves. The gut is the organ system that can lose blood for months in complete silence, and that silence is exactly why the finding is chased.

What does "unexplained" mean, and why does it depend on who you are?

"Unexplained" means there is no obvious, sufficient reason on the table for the iron to be gone. The weight the finding carries depends heavily on whether a person has a large, routine, non-GI route for losing blood. That is not a technicality — it is the single fact that most changes what happens next, and it is why two people with identical lab numbers get very different conversations.

In men. There is no physiologic route for a man to lose blood monthly. Iron deficiency in an adult man is, by default, unexplained, and the gut is the default place to look. This holds regardless of how healthy he feels or how normal his stool looks.

In postmenopausal women. Once menstruation has stopped, the same logic applies. The route that used to explain iron loss is closed, and a new explanation is needed.

In people who menstruate. Heavy periods genuinely can drain iron, and often do. The trap is treating that as an automatic answer. A period explains iron loss; it does not prove the iron loss came only from there. A person can menstruate heavily and have a colon polyp at the same time — the two are unrelated conditions and neither protects against the other. When bleeding is heavy enough to be the plausible whole story, that is a judgment call a clinician makes with the person, not a box that closes itself.

Occult bleeding means blood loss small enough that it is not visible in the stool and produces no symptom at the time it happens.

In older adults of any sex. Age raises the prior probability of the findings that matter, which is why an unexplained deficiency in an older adult tends to move faster through the system rather than slower.

The honest summary is that "unexplained" is a clinical judgment about whether the available explanation is big enough to account for what the labs show. It is worth asking directly: does my clinician think this is fully explained, or are we assuming it is?

Can a bleed really be invisible?

Yes, and this is the fact that most surprises people. The amount of blood needed to make stool visibly bloody or black is far more than the amount needed, day after day, to outpace what the diet replaces. A bleed can be too small to change the color of anything and still empty the body's iron stores over months. Normal-looking stool rules almost nothing out.

Think of it as a slow leak in a tire rather than a puncture. A puncture is loud and immediate — you know. A slow leak does nothing you can perceive, and you find out from a gauge weeks later. The blood count is the gauge. It is often the first and only instrument that registers the leak.

This is also why "I would have seen it" is not reassurance, even though it feels like it should be. Blood from higher up — the stomach or small intestine — gets broken down by digestion on the way through and stops looking like blood at all. Blood from lower down can be mixed thoroughly enough into formed stool to be invisible. Neither situation involves any pain, urgency, or change in bowel habit that would prompt a person to look in the first place.

A great many people with a hidden bleed feel completely well, which is precisely why the lab result does the talking.

What people often do notice, in retrospect, is nothing dramatic: getting winded on stairs that used to be nothing, an unfamiliar fatigue that got blamed on work or age, feeling cold, a dulled appetite. These are the body running short of oxygen-carrying capacity, and they arrive so gradually that most people adjust rather than notice. It is common for someone to only recognize how tired they had become after the anemia is corrected.

What the evaluation usually looks like

The standard approach to unexplained iron deficiency is to examine both ends of the gastrointestinal tract, because either end can be the source and the labs cannot tell them apart. In practice that means an upper endoscopy to look at the esophagus, stomach, and the start of the small intestine, plus a colonoscopy to look at the colon — often arranged together, under one sedation, in one appointment.

Bidirectional endoscopy is the term for doing an upper endoscopy and a colonoscopy in the same session.

The logic behind looking upward is well established in guideline language: in people being evaluated for upper-gut symptoms, alarm features — including bleeding, unintentional weight loss, and trouble swallowing — are what turn an empiric-treatment approach into an endoscopy, as is being 60 or older 1. Iron deficiency belongs in that family of findings that argue against treating first and asking later.

The logic behind looking downward is the same in reverse. When bleeding from the lower gut needs an explanation, colonoscopy is the primary diagnostic test, performed after bowel preparation so the lining can actually be seen 2. A colonoscopy done for anemia is a diagnostic exam with a specific question, not a routine screening exam that happens to be on the calendar.

What they are looking for. The list is broad and most of it is benign: a stomach or duodenal ulcer, inflammation from medication, an area of irritated lining, a vascular malformation, a polyp, inflammatory bowel disease, celiac disease, or a tumor. Finding a benign source is the common outcome. Finding a cancer early, before it ever caused a symptom a person could feel, is the outcome the whole exercise exists to make possible.

If both exams are clean. Sometimes they are, and the source is in the small intestine — the long middle stretch that neither scope reaches. There are further tests for that stretch, and a normal pair of scopes is a reason for the next conversation rather than the end of one.

Which symptoms change the timeline

Iron deficiency by itself is a finding that gets worked up in weeks, not hours. But some things travelling alongside it move the clock, and they are worth naming plainly rather than leaving to intuition. The presence of any of these turns a scheduled evaluation into a prompt one, and a few of them turn it into a same-day one.

Findings that mean prompt medical evaluation rather than a routine appointment:

  • Rectal bleeding or visible blood in the stool. Bleeding is on the standard list of digestive warning signs that prompt evaluation rather than watchful waiting 3.
  • Constant abdominal pain, vomiting, or an inability to pass gas or stool 3.
  • Unintentional weight loss. This one carries independent weight: it is a recognized red flag whose leading causes include malignancy — including cancers of the gut — as well as non-malignant GI disease, and it warrants a workup for serious disease in its own right 4.
  • A family history of colorectal cancer, which changes the calculus on how hard to look and how early 3.

Findings that mean emergency care now, not an appointment: vomit that looks like coffee grounds, black tarry stool, large volumes of red blood, passing clots, or feeling faint, gray, or breathless. Hematochezia — visible red blood — accompanied by signs that the circulation is struggling can indicate a brisk bleed from higher up in the gut and is evaluated as an emergency, sometimes with upper endoscopy rather than colonoscopy first 2.

The wider question of what belongs in an emergency department versus a clinic visit — emergency, urgent, or can it wait — is a genuinely difficult judgment, and coffee-ground vomit sits firmly on the emergency side of it. So does the point at which visible bleeding becomes heavy: when rectal bleeding becomes an emergency is a matter of volume, speed, and how the person feels, not of whether it hurts.

What drains iron that is not a gut bleed

Not every iron deficiency is a bleed, and a good evaluation considers the alternatives rather than assuming. The point of naming them is not to supply a reassuring story to settle on — it is that these are the possibilities a clinician is weighing while deciding how hard to chase the gut. Most of them are identifiable, and several are found with a conversation and a blood test rather than a procedure.

Menstrual blood loss. The most common non-GI route in people who menstruate, and often the true answer.

Pregnancy and breastfeeding. Iron demand rises substantially to build a second circulation. Deficiency here is expected physiology rather than a mystery.

Malabsorption. Iron has to cross the lining of the upper small intestine to get in. Conditions that damage that lining, celiac disease among them, can produce iron deficiency without any blood being lost at all. This is one reason the upper endoscopy is not just a search for a bleeding site — the same exam can sample the lining.

Prior gut surgery. Operations that bypass or remove the stretch of small intestine where iron is absorbed reduce how much can get in, regardless of intake.

Blood donation. Frequent donation over years draws down stores — a real cause, and an easily missed one, because it does not feel like blood loss.

Diet. Genuine dietary iron deficiency happens, particularly with restricted diets and high demand. It is a thinner explanation than it sounds in an adult eating variedly, and clinicians are appropriately reluctant to accept it as the whole story without ruling the gut out first.

A plausible alternative explanation is not the same as a proven one, and the gut is not excluded by the existence of another suspect.

Medicines that make a quiet bleed more likely

Some common medications raise the odds that the gut lining is bleeding slowly, and they belong in the conversation because they are so easy to forget. Over-the-counter drugs especially go unreported — people list their prescriptions and omit the ibuprofen they take most days, because it does not feel like a medication.

Anti-inflammatories. NSAIDs — ibuprofen, naproxen, and their relatives — can irritate and erode the stomach and duodenal lining. The damage is often silent, which is the theme of this whole page.

Aspirin. Including low-dose aspirin taken for the heart. It thins the blood and irritates the lining, a combination that makes a small erosion bleed more than it otherwise would.

Anticoagulants and antiplatelet drugs. These do not usually create a lesion. They make one that already exists bleed more, for longer. An anticoagulant that unmasks a bleed has arguably done a person a favor by making a hidden lesion announce itself — but the lesion still needs finding.

Acid-suppressing drugs. Proton pump inhibitors sit on the other side of this ledger. Guideline advice on stopping them draws a line worth knowing: people without a clear ongoing indication can reasonably attempt to step down or stop, while those with specific indications — erosive esophagitis, Barrett's esophagus, or an indication based on bleeding risk — are the ones who should stay on them 5. Someone taking an NSAID or aspirin long-term for a good reason may be on acid suppression precisely because of that bleeding risk.

None of this is a reason to change anything unilaterally. The medication list is diagnostic information — it tells a clinician which lesion to expect and where. Bringing the actual bottles, including the ones bought off a shelf, makes that list accurate in a way memory usually does not.

Why iron alone does not close the question

Iron replacement fixes the number. It does not fix the reason the number moved. This is the most consequential misunderstanding on this topic, because iron works — the hemoglobin climbs, the fatigue lifts, the person feels genuinely better — and feeling better is powerfully persuasive that the problem has been dealt with. It hasn't. It has been compensated for.

If a lesion is bleeding, replacing iron means pouring in supply to match a loss that is still running. The lab result improves while the underlying cause carries on unexamined. Where that cause is a polyp or an early tumor, the months bought by the supplement are months during which the thing that was findable and treatable got less so. The supplement is not the error. Stopping at the supplement is.

Iron treats the anemia. The evaluation treats the reason for it. They are different jobs and both need doing.

There is a practical wrinkle worth knowing: oral iron commonly turns stool dark or black, which can be mistaken for the black tarry stool of an upper gut bleed and can also make it harder to interpret what is being seen. This is a known and manageable overlap, and it is a reason to mention iron supplementation to anyone assessing stool color rather than to assume they know.

The question worth asking out loud. If a low iron level has been found and the plan is a supplement, the reasonable question is simply: what do we think is causing this, and are we going to look? A clinician has usually already thought about it. Sometimes, in a short appointment about something else, nobody has — and the anemia found incidentally on a pre-operative panel is exactly the one treated as a footnote.

For an adult past the age at which the colon is routinely examined anyway, the two conversations converge. Bleeding after 45 already argues for a colonoscopy on its own terms; an unexplained iron deficiency in the same person argues for one whether or not anything visible has ever appeared.

Common questions

No. The amount of blood needed to visibly change stool color is much larger than the amount needed to drain iron stores over months. Blood from the stomach or small intestine is also digested on the way through and stops looking like blood. Normal stool is compatible with a slow bleed, which is the main reason the lab result is trusted over appearances.

Feeling well is typical, not reassuring. Anemia develops slowly enough that most people adapt without noticing, and a small gut bleed produces no sensation at all because the gut lining has no pain fibers reporting slow oozing. They can explain iron loss, but they do not exclude a second source. The two conditions are unrelated and neither protects against the other. Whether menstrual loss is a sufficient whole explanation is a judgment made with a clinician, weighing how heavy the bleeding actually is against the degree of deficiency and any other findings.

Typically an examination of both ends of the gut: an upper endoscopy for the esophagus, stomach, and start of the small intestine, and a colonoscopy for the colon, often in one session. Blood tests may also look for causes of poor absorption, such as celiac disease. If both exams are clean, the small intestine is the next stretch to consider.

Iron corrects the anemia but does not address why the iron was lost. If something is bleeding, supplementation replaces supply while the cause continues unexamined, and the improvement in how a person feels can be mistaken for resolution. Iron and the evaluation are two separate jobs, and the supplement does not substitute for finding the source.
