For a commercially insured client the question is contractual. Whether your agreement obliges you to collect the plan's cost-share, and whether a published sliding scale changes the fee the plan is billed against, are answered in its fee and cost-share clauses. Read them before setting a policy, and take a clause you cannot parse to a health care attorney.

- a quarterly pull of your claims by code and by units, sorted high to low
