MBHO carve-outs: finding who actually pays for therapy
Summary
Behavioral health is often managed by a different company because your medical plan carves the mental-health and substance-use benefit out to a managed behavioral-health organization, or MBHO, with its own network, fee schedule, authorization rules, claims address, and payer ID. To find who actually pays, read the behavioral-health line on the member's card, verify benefits with that entity, and confirm which company adjudicates each code — because commercial, Medicare, and Medicaid carve-outs each route differently.

Why is behavioral health managed by a different company?
Because the medical plan usually does not administer the behavioral-health benefit itself. It carves that benefit out to a managed behavioral-health organization — an MBHO — that runs a separate network, sets its own fee schedule, applies its own authorization rules, and receives claims at its own address under its own payer ID. Optum, the behavioral arm of one of the largest carriers, publishes exactly this kind of separate join-the-network, authorization, and claims portal for providers 1Ref 1Optum Behavioral Health (2026).Provider Express. That a behavioral benefit is commonly administered through a separate MBHO's own join-the-network, authorization, and claims portal, cited as that payer's published example.. The card in the client's wallet may show one brand while a different company actually pays you.
What a carve-out actually changes for you
A carve-out changes almost every operational touchpoint, which is why claims sent to the medical plan bounce. Under a behavioral carve-out you typically credential separately with the MBHO, submit to a different payer ID, request authorization through a different portal, and bill against a different fee schedule than the medical benefit uses. The member's ID card often prints a separate behavioral-health phone line and a separate claims address for exactly this reason — the medical processor never sees the therapy claim.
- Separate credentialing: being in the medical network does not put you in the behavioral network.
- Separate payer ID and claims address: use the routing on the card's behavioral line, not the medical one.
- Separate authorization and fee schedule: the MBHO's rules and rates, which may differ from the medical plan's.
Three carve-out patterns: commercial, Medicare, Medicaid
The routing depends on the coverage type, and each pattern behaves differently. In commercial coverage, the medical carrier subcontracts the behavioral benefit to an MBHO. In Original Medicare, there is no carve-out: Medicare covers behavioral services directly and, since 2024, enrolls and pays marriage and family therapists and mental-health counselors alongside the longer-recognized professions 2Ref 2Centers for Medicare & Medicaid Services (2025).Medicare and Mental Health Coverage. That Original Medicare covers behavioral services directly with no carve-out and, since 2024, enrolls and pays MFTs and mental-health counselors.. In Medicaid, states commonly carve behavioral health out to a managed behavioral plan or a county authority; California's Medi-Cal, for one, splits behavioral health across managed-care plans and county mental-health plans, and the state's Department of Health Care Services publishes which services route where 3Ref 3California Department of Health Care Services (2026).California Department of Health Care Services. That California's Medicaid program administers behavioral health through managed-care plans and county mental-health plans, as published by the state agency..
| Coverage type | Who administers behavioral health | Where the claim goes |
|---|---|---|
| Commercial | A carved-out MBHO under the medical carrier | The MBHO's payer ID and claims address |
| Original Medicare | Medicare directly, no carve-out | Your Medicare Administrative Contractor |
| Medicaid | Often a managed behavioral plan or county authority | The state's designated behavioral payer |
Finding who actually pays: the verification sequence
Do this before the first visit, every time, because the brand on the card is not proof of who adjudicates. Call the behavioral-health number printed on the member's card — not the medical line — and capture four things: the payer ID and claims address, whether authorization is required and how to get it, the covered services and any carve-out details, and the effective benefit for the specific psychotherapy code you plan to bill 4Ref 4APA Services, Inc. (2025).Psychotherapy Codes for Psychologists. That the psychotherapy CPT family — timed individual, family, and group codes — each carries its own coverage, so benefits must be verified per code.. Verify eligibility on the date of service, and confirm which entity adjudicates each code you expect to use.
The timed individual codes, the family codes, and the group code each carry their own coverage and rate, so a benefit check that stops at a single code number leaves you guessing on the rest of the session mix.
The exception that trips people up: collaborative care bills the medical benefit
One important service runs the other way. When a medical practice delivers behavioral health integration or collaborative care, those care-management codes are billed by the treating medical practice to the medical benefit — not to the behavioral carve-out — because Medicare and commercial plans pay them as part of the medical, primary-care-anchored benefit 5Ref 5Centers for Medicare & Medicaid Services (2024).Behavioral Health Integration Services. That behavioral health integration and collaborative care codes are billed by the treating medical practice to the medical benefit, not to a behavioral carve-out.. A solo behavioral clinician acting as the psychiatric consultant in that model gets paid by the medical practice under its arrangement, not by submitting a carve-out claim of their own.
That single distinction resolves a common denial: a collaborative-care code routed to the behavioral MBHO will not pay, because the MBHO does not administer it.
When it looks like a carve-out but is not: EAP and court-ordered work
A few adjacent arrangements route like carve-outs but are not the medical benefit at all, and treating them as ordinary insurance is how claims get denied. Employee assistance program EAP work is paid by the EAP vendor on its own short-term authorization, frequently through the same behavioral-health portals a clinician already uses 1Ref 1Optum Behavioral Health (2026).Provider Express. That a behavioral benefit is commonly administered through a separate MBHO's own join-the-network, authorization, and claims portal, cited as that payer's published example., and is billed to the EAP rather than to the health plan. Court-ordered treatment is another trap: an order to attend therapy does not create coverage, so who pays still turns on the client's actual benefit or their own pocket.
In both cases the operational move is the same — confirm the paying entity, the authorized session count, and the rate in writing before the first session, not after the claim rejects.
