Asking about firearms directly is part of a competent assessment, not an overstep; means access changes the risk calculus more than almost any other single factor. Record what you asked and what the client reported, in their words where you can, so the note carries the assessment rather than a summary of it.
Distinguish ideation from imminent danger
Passive ideation, active ideation without plan or intent, and imminent danger are different clinical situations calling for different responses. Most clients with suicidal ideation can be managed as outpatients with a safety plan and closer follow-up; a smaller number, with intent, a plan, access to means, and few protective factors, need a higher level of care today. The decision is clinical and turns on capacity for safety between now and the next contact, not on any single answer.
Protective factors — reasons for living, a reachable support person, engagement with you, no access to a lethal method — can shift an ambiguous picture toward outpatient management, but they do not cancel active intent with a plan. When the picture is a minor in crisis, the parents or guardians and their consent enter the calculus, and the analysis changes.
Build the safety plan in the room
For a client who can stay safe as an outpatient, the safety planning intervention is the core clinical act of the session. The means-restriction step is where you translate the assessment into action: agree on securing firearms and limiting or locking medications, and name who will hold them.
The plan is only useful if the client can reach it; give them a copy in a form they will keep and file an identical copy in the record. If this same crisis happens on video rather than in the room, the mechanics of crisis at a distance change — you have to know where the client physically is before you can send anyone.
When it is imminent: activating emergency response solo
When the assessment shows imminent danger and the client cannot keep themselves safe, the session becomes an emergency and outpatient tools are no longer enough. Hospitalization may be voluntary or, where the client meets your state's criteria and will not accept care, involuntary; the standards and who may initiate a hold are set by state law, so know your state's process in advance rather than reading the statute during the crisis. If the crisis follows an overdose, medical stabilization comes first — what happens after an overdose is a medical emergency before it is a psychiatric one.
Document the crisis session
The crisis note is written the same day and shows your reasoning, not just your conclusion. Record the risk assessment and the tool you used, the specific findings, the protective and risk factors you weighed, the safety plan you built, the means-restriction agreement, the level-of-care decision and why, everyone you contacted, and the follow-up you arranged. A note that says the client denied intent and was safe to leave, with nothing behind it, is the note that does not hold up.
Keep the crisis note in the clinical record, not only in your private psychotherapy notes, since you may need to produce it and act on it. Contemporaneous beats complete-but-late; if you must finish it after hours, note the time you wrote it and why.
Afterward: follow-up and support for you
Before the client leaves, close the loop: confirm the next contact, make it soon, and consider a caring follow-up message between sessions. If you referred or transferred, do a warm handoff rather than a phone number on a card, and document the receiving contact.
