96127: getting paid for the screens you already score
Summary
96127 is the CPT code for a scored, standardized emotional or behavioral screen — the PHQ-9 or GAD-7, for example — reported in addition to the visit's psychotherapy or evaluation code, usually one unit per instrument. Getting paid means documenting the actual scored instrument, linking it to a supported diagnosis, and staying inside each payer's units-per-day and frequency limits. It is not a care-management code, and it is not a replacement for the visit itself.

What 96127 actually pays for
96127 is the CPT code for a brief emotional, behavioral, or developmental assessment administered with a standardized instrument, scored, and documented — the PHQ-9 for depression or the GAD-7 for anxiety being the two most common in an adult behavioral-health practice. It pays for the structured screen itself, reported in addition to the psychotherapy or evaluation code for the visit, and you can generally report it more than once when more than one instrument is used.
The PHQ-9 is a validated nine-item depression measure scored from 0 to 27, with severity bands rising at 5, 10, 15, and 20 and a score of 10 or higher used as the screening threshold for major depression 1Ref 1Kroenke K, Spitzer RL, Williams JB (2001).The PHQ-9: validity of a brief depression severity measure. The PHQ-9's defining validation — a nine-item depression measure scored 0-27 with severity bands and a threshold of 10 for screening — the instrument 96127 most often pays to administer and score.. The GAD-7 is a seven-item anxiety measure scored 0 to 21 with a comparable 10-or-higher threshold 2Ref 2Spitzer RL, Kroenke K, Williams JB, Löwe B (2006).A brief measure for assessing generalized anxiety disorder: the GAD-7.The GAD-7's defining validation — a seven-item anxiety measure with a screening threshold of 10 — a second common instrument billed under 96127.. 96127 pays for administering and scoring an instrument like these; it never asks you to reproduce the items, and the items themselves do not belong pasted into the chart.
96127 is not a formal psychological-testing code, and it is not the visit itself. It is the small, specific code for a scored screening instrument, and it lives or dies on whether that scored instrument is in the record.
How you report it alongside the visit
96127 rides on top of the visit, not instead of it. On the claim you report the code that anchors the encounter — a 90791 diagnostic evaluation, a timed psychotherapy code such as 90832, 90834, or 90837, or an evaluation-and-management code — and add 96127 for each instrument you scored 3Ref 3APA Services, Inc. (2025).Psychotherapy Codes for Psychologists. The psychotherapy CPT family (90791 evaluation; 90832/90834/90837 timed therapy) that anchors the visit 96127 is reported in addition to.. Report one unit per instrument, put the screen's diagnosis on that line, and be ready to append a distinct-service modifier if the payer bundles it against the visit.
On a CMS-1500 that means the primary service on its own line and 96127 on its own, each pointing to the diagnosis that justifies it. Place of service and who administers the screen both matter: a screen you hand a patient and score yourself bills cleanly under your own NPI, while one a staff member administers pulls in the supervision questions the next section covers. The screen is fast to deliver and easy to under-document, so the discipline is on the note, not the claim.
Who can bill 96127
Whether you can bill 96127 at all turns on your license type and the payer's rules, not on the code itself. Medicare, which since 2024 recognizes marriage and family therapists and mental health counselors as billing providers for its mental health benefit, sets one set of conditions; commercial payers set their own 4Ref 4Centers for Medicare & Medicaid Services (2025).Medicare and Mental Health Coverage.Medicare's behavioral health coverage and billing-provider eligibility, including its 2024 recognition of MFTs and mental health counselors, which governs who may bill a screen.. In a group, watch the incident-to in behavioral health rules that govern billing a screen administered by support staff under a supervising clinician.
As a solo clinician billing under your own NPI, incident-to usually is not your question — you administered and scored the instrument yourself. It becomes one the moment an assistant or a trainee does the administration, because then the rules about who performed the service, and under whose supervision, decide whether the line is billable at all.
When the screen belongs to a different code family
If you run structured, repeated symptom measurement inside an integrated or collaborative-care program, the rating scales may belong to a different code family than 96127. Medicare's behavioral health integration and collaborative care management codes — 99484 for general care-management and the CoCM 99492–99494 set — pay for that ongoing program tracked over time, rather than for a single scored screen 5Ref 5Centers for Medicare & Medicaid Services (2024).Behavioral Health Integration Services. That Medicare's BHI/CoCM codes (99484, 99492-99494) pay for programs built around validated rating scales tracked over time — the code family distinct from a single 96127 administration.. Billing 96127 on top of those codes for the same measurement double-counts the work.
This is the measurement-based care distinction: the integration codes are built around validated rating scales followed longitudinally, whereas 96127 pays for the discrete administration. Route a given instrument through one lane or the other per encounter, not both, and pick the lane your actual workflow supports.
Frequency, units, and why 96127 denies
The most common denials are for units and frequency, both payer-specific. A payer may allow only so many units of 96127 per date of service or cap how often it is reimbursed, and those limits live in each plan's own published policy — Optum's Provider Express, for its network, is one place that policy is posted 6Ref 6Optum Behavioral Health (2026).Provider Express. One large behavioral-health network's own provider portal, as an example of where a payer publishes its 96127 units-per-day and frequency policy.. Link each screen to a supported diagnosis, and expect Z-codes for pure screening to draw more scrutiny than an active F-code.
When a screen denies, the fix usually lives in the same three places:
- Units: did the count exceed that payer's per-date cap for the code?
- Diagnosis: does a supported diagnosis point to the 96127 line?
- Bundling: did the screen need a distinct-service modifier against the visit?
Work denials and appeals from the payer's own policy language, and note that some state Medicaid programs route screening through their own H-codes instead of 96127 entirely.
Documentation that survives an audit
An auditor asks one question of a 96127 line: does the chart hold the actual instrument, scored? Keep the named instrument, the date, the numeric result, your interpretation, and one line on how the result moved the plan — a medication change, a therapy focus, a referral. Note who administered and scored it. A code with no scored instrument behind it is the classic recoupment target.
The scored instrument in the note is what makes a 96127 line defensible. A tight record for each screen holds:
- The instrument named and dated, with the total score recorded.
- Your short interpretation of what the score means for this patient.
- The diagnosis the screen supports, pointed to on the claim line.
- One sentence connecting the result to a decision in the plan.
- Who administered and scored the instrument, if not you.
